NY Podiatrist Warns of Risks of Wearing Snow Boots
It’s important for every New Yorker to be cautious of the foot and ankle problems that can be caused or worsened by wearing snow boots,” Rock G. Positano, DPM, a podiatrist for the Hospital for Special Surgery, told The Post. “Since there are more risks than rewards with snow boots, these shoes certainly aren’t made for walking.” Positano noted that while snow boots appear “safe,” they offer little to no orthopedic support. “This leads to tendon and ligament overuse, which can cause painful injuries, especially when making long treks in the snow or in the rain,” he explained.

“An influx of knee and back problems can also arise from the lack of support, mixed with the presence of underlying orthopedic problems.” In addition, Positano shared that snow boots are not as well insulated as we might assume, and this lack of protection can lead to skin issues such as frostbite and ulceration. “The likelihood of these can increase if you also have sensitive skin,” he added, “so making sure you have footwear that can handle harsh weather but accommodate any skin conditions is vital.”

Query: Use of CPT 97760 / 97763 for Custom Orthotics
Is it appropriate to use code 97760 (Orth Train/Mgt, initial encounter) for an orthotic pick-up appointment and code CPT 97763 (Orth Train/Mgt, sub encounter) for 4-6 week orthotic check/re-eval? Are you aware of any payers paying these codes and do they all require the -GP modifier?

Response from Codinghelpline.com: Those codes are primarily for PT or OT so the -GP modifier and plan of care are both required. Also, if you read the CPT, it is for one unit, I believe, of 15 minutes. When was the last time you spent 15 minutes training and fitting a patient with a brand new foot orthotic?
This code is primarily for fitting and dispensing an AFO or prosthesis when you are training a patient to use this device. Think of the post CVA patient with a drop foot who needs to rehab to use an AFO and needs to be shown how to use the AFO and a cane. I think it is a big overstretch to use any training code with a foot orthotic, especially if the orthotic is not covered.

Brooke Bisbee, DPM has decided, based on which stakeholders she hears, to reject ACGME’s offer to explore accreditation of DPM residencies. PM News’ survey overwhelmingly displayed APMA’s membership would prefer a DO or MD degree to our current DPM degree. Instead, podiatry will focus on student recruitment without ever mentioning that DPMs would prefer a different degree. This maneuver is something I call a serious moral and ethical offense. Let me know when the survey results are included in any information forwarded to potential students. There seems to be uncertainty about ACGME’s involvement. Rather than explore reservations and come up with a solution, we slam the door on ACGME’s toes, generating more trauma for the profession. There is a solution.
A true leader would propose to ACGME that they send a couple of their accreditation team members along with CPME’s re-accreditation of two podiatry programs. One program would be at a university hospital-based podiatry residency, and the other program would be a community hospital-based podiatric residency. The two experienced ACGME accreditors would submit their evaluations for the two programs. These evaluations would have no bearing on the accreditation of the podiatry programs but would be open to all APMA members to see.
As a sign of good faith, ACGME would invite a couple of educationally savvy DPMs on a re-accreditation visit to two of the several ACGME-accredited Foot and Ankle Fellowships, and the DPM evaluation would proceed exactly like the DPM residencies.
