PODIATRISTS IN THE NEWS
TX Podiatrist Saves Feet and Lives at New San Antonio Clinic
Michael Sobolevsky, DPM is one of the leaders of the new Limb Salvage Clinic at University Health’s Texas Diabetes Institute in San Antonio, which opened last year. It’s a collaboration with Dr. Anand Prasad, founding director of the University Health Cardiovascular Center, to address the rising number of diabetes-induced amputations in Bexar County. With their combined expertise, the two are working to provide holistic care for patients with diabetes and vascular disease. Rather than relying on amputation, the doctors offer their specialties under one roof to detect and treat limb-threatening conditions early and address patients’ overall cardiovascular risk.
Dr. Michael Sobolevsky
Dr. Sobolevsky attended medical school at Rosalind Franklin University in Chicago and completed his residency at UT Health San Antonio in 2018. He ultimately chose podiatry because of the range of options it provided him in private practice, a hospital-based system, clinical or surgical. Since starting his own podiatry division at the Texas Diabetes Institute, it has grown into a team of five.

PODIATRISTS IN THE COMMUNITY
MN Podiatrist Volunteers at Essentia Clinic
There are a handful of Essentia team members involved in what can best be described as a passion project. They include family medicine physician Dr. Stephen Smith, podiatrist Robert Sheckler, III, DPM, and registered nurse Kelly Nelson. Caring for their community is what they do. Nelson started the remarkably popular podiatry program at Project Care in Virginia, MN. She and Dr. Sheckler clip toenails and provide other podiatry services, the demand for which results in lines snaking out the door.
Dr. Robert Sheckler
Dr. Lisa Seeber, a family medicine physician at the Essentia Health-Virginia (MN) Clinic, has volunteered at Project Care for years. Dr. Seeber called it the “most important thing” we offer. “I think Dr. Sheckler and Kelly are doing the work of angels,” Dr. Seeber says. “What a gift to the community. We’re so lucky to have them.”

PRACTICE MANAGEMENT TIP OF THE DAY
9 Ways to Smooth Collections at Your Practice
Here are nine actionable strategies to help your practice get paid faster, reduce write-offs, and keep patients satisfied in the process.
8. Tighten your claims process to reduce denials
Denied claims are among the biggest drags on practice revenue, and the trend is moving in the wrong direction. Industry data indicate that initial claim denial rates have climbed steadily in recent years and may now sit in the range of 12% to 15% for many organizations, according to Medical Economics. Investing in regular coder training, using claim-scrubbing software that catches errors before submission, and building a systematic denial management process can keep your clean claims rate above 95%, the benchmark that top-performing practices target. Every point of improvement in that rate translates to real revenue recovered.

PODIATRIC SUPERGROUPS IN THE NEWS
Upperline Health Enters VA Market with Acquisition of Coastal Podiatry, Inc.

Upperline Health is proud to announce the May 21, 2026 acquisition of Virginia-based Coastal Podiatry, Inc. Upperline Health is one of the nation's largest specialty care providers, delivering whole-person care in specialty settings.

10 Financial To-Do's for Spring
Spring is a great time to check in with your finances to make sure everything is in good order. Completing these 10 tasks might seem like a lot of work, but the payoffs are worth it.
1. Adjust your withholding taxes if necessary
If you've completed your 2025 tax returns and discovered you're getting a big refund, you might want to slightly reduce your W-2 withholding rates. While it's always nice to get a big chunk of your tax payments back, it might be better for you to give less to Uncle Sam throughout the year.
Likewise, if you ended up owing taxes for the year, you might want to increase your withholding rates. A tax professional can help figure out your "tax Goldilocks zone."

ON THE LECTURE CIRCUIT
IL Podiatrist Delivers Keynote on Peptide Therapy
Sev Hrywnak, DPM, MD, a board-certified physician in functional and regenerative medicine, delivered a standout presentation at the recent Chicago Biohackers Regenerative Conference, captivating clinicians and researchers with cutting-edge peptide science. The keynote highlighted novel peptide sequences and their mechanism of action in tissue regeneration, emphasizing safety, efficacy, and translational potential. Attendees were encouraged by the robust pre-clinical data and early clinical signals suggesting accelerated healing in challenging cases.
Dr. Sev Hrywnak
Key takeaways included targeted peptide applications for musculoskeletal repair, skin regeneration, and inflammatory modulation. Dr. Hrywnak shared several exciting research articles, including studies demonstrating enhanced collagen synthesis, improved angiogenesis, and reduced scar formation in pre-clinical models. “Peptide therapy will become a mainstay for podiatric physicians and surgeons,” stated Hrywnak.

Treating Female Athletes
Here are some valuable tips that have aided Savannah Santiago, DPM in navigating her experiences as a student and a resident:
Conclusion
Understanding the Female Athlete Triad and RED-S isn’t just an academic exercise—it’s essential to providing safe, responsible, and compassionate care to the athletes who trust us. These athletes often present with problems that land squarely in our offices: stress fractures, chronic overuse injuries, delayed healing, and bone stress reactions that don’t match their training load. But beneath those injuries is often a deeper story—one shaped by pressure, perfectionism, misinformation, and a culture that has historically valued performance over health.
As clinicians, we are in a unique position to interrupt that cycle. By recognizing the early signs of low energy availability, asking the right questions, and approaching these conversations with empathy rather than judgment, we can change the trajectory of a young athlete’s life. Effective treatment isn’t just about diagnosing a stress fracture; it’s about helping athletes rebuild their relationship with food, training, and their bodies. It’s about creating an environment where health is prioritized and where performance is fueled—not restricted.
The more we educate ourselves and one another, the more empowered we become to advocate for these athletes. And ultimately, that advocacy is what protects them, keeps them competing safely, and allows them to thrive long after their competitive years are behind them.

PRACTICE MANAGEMENT TIP OF THE DAY
6 Ways to Cut Denials at Your Practice
Most claim denials are preventable. Master these six strategies to stop revenue leaks, speed up payments, and keep your staff focused on patient care.
3. Improve clinical documentation quality
Insufficient documentation drives a significant portion of denials. Payers reject claims when documentation doesn't adequately support billed services, required documents are missing, or diagnosis codes don't justify medical necessity. The issue often stems from communication gaps between providers and coding staff. A patient may receive a covered service, but an incorrect or incomplete diagnosis code makes the claim appear not medically necessary.
Effective practices hold regular in-services to educate physicians about payer changes and documentation requirements. Using artificial intelligence (AI)-powered coding audits helps identify documentation patterns that trigger denials, allowing practices to correct behaviors before claims go out the door. Consider implementing documentation prompts in your EHR that remind providers to include necessary details. Complete documentation should specify the diagnosis, procedure details, and patient history that justify the services billed.

RESPONSES/COMMENTS (THE FUTURE OF PODIATRY) - PART 1
RE: Immediate Need to Address Podiatry’s Future
From: Joseph T. Hogan, DPM
It was my pleasure to attend virtually the St Louis Podiatry Seminar this past Friday and Saturday. The content and presentation of the lectures were excellent. I also found the presentation by Rod Tomczak, DPM, MD, EdD to be very informative. I have been in practice for 50 years. When I began practice in Binghamton, NY, there were two DOs in town who did not have hospital privileges. There were no FNPs and PAs. That has all changed.
I am board certified by ABPM, ABFAS, ABQAURP, faculty of a medical school, faculty of a number of schools of podiatric medicine, and faculty of a family practice residency program. Today, we, as DPMs, and they as DOs, FNPs and PAs all have hospital privileges in significant numbers. The numbers of applicants to our three-year residency program are minimal and in fact threaten the continued existence of our hospital-based residency program.
I believe our profession needs to consider establishing a collaborative study group regarding our immediate future. I recommend that Dr. Tomczak be a member of that body. The future of our profession is now, not five years from now. We need to ensure the continued existence of our specialty. We, as podiatrists, can ensure the future continued existence of our specialty with an enhancement of our degree to include us as a recognized podiatric physician.

RESPONSES/COMMENTS (THE FUTURE OF PODIATRY) - PART 2A
From: H. David Gottlieb, DPM
In all my years as a podiatrist, I do not recall having, but imagine that I have met Dr. Jacobs, but I have heard only good things about him. In his latest post on the future and standing/prestige of podiatry in today's medical world, he eloquently stated what I have been saying: "You are responsible for your own fate and circumstances."
For my first 20 years, I performed hammertoe, distal bunion, and toenail surgeries. Nail debridements and lots of the old C&C (corns, calluses). My patients were thankful for the relief from pain and the ability to continue their chosen path. Several tracked me down years later to express their gratitude. I could see that this was a pathway to financial reward. Despite what Dr. Jacobs states, a good living can be made performing this vital service IF...

RESPONSES/COMMENTS (THE FUTURE OF PODIATRY) - PART 2B
From: Elliot Udell, DPM
Dr. Jacobs is correct when he asserts that there are many podiatrists who do Charcot foot surgery, total ankles, and have great surgical and podiatric medical practices and make a lot of money. Let's also focus on the other example he gave. At the dinner meeting he addressed, only four of the 40+ attendees were doing anything more than cutting nails all day.
Our profession's problem is to find ways to increase enrollment in our schools and to ensure that the public will be able to avail themselves of our services not just now, but fifty years from now. Let's be true to ourselves. A pre-med student can choose the route of being an MD, DO, DDS, or a DPM. Since they are putting their lives and future finances on the line, these students will be visiting doctors' offices. How do we make sure that they visit the offices of Dr. Jacobs and his students or the 36+ podiatrists at the dinner meeting who only cut toenails and have empty offices? If we solve this problem, the problem of insufficient applicants at our schools will automatically be solved.
Elliot Udell, DPM, Hicksville, NY
RESPONSES/COMMENTS (MEDICARE ISSUES)
RE: Why is Medicare Enrollment Getting Tougher?
From: Paul Kesselman, DPM
I know that most if not everyone reading this has never had a run-in with the OIG. Most if not all here are honest and ethical physicians and manufacturers or represent them. But the ongoing list of unethical criminals who continue to prey on the Medicare Trust Fund is sickening and we all should be appalled. Reading today’s OIG report , one can easily see why the Trump administration has put a standstill to most DME enrollment or at least made it tougher. Another method by which to stop DME fraud would be to put a stop to all mail order delivery braces. Although I am sure that wouldn’t stop all the fraud in this category, it would certainly cause a sizeable reduction.
CMS needs to do something other than place a moratorium on DME enrollments and post-payment prosecutions. Putting more expensive law enforcement efforts after the money has been deposited in the unscrupulous suppliers’ bank accounts is putting good money after bad. Good luck getting that money back once it's safely transferred to an offshore account.
Widespread pre-payment audits for all new suppliers is one idea. Another is to curtail payments of those whose reimbursements increase exponentially by a certain amount. I am still waiting for an answer from the Feds after a lecture on fraud investigations over twenty years ago.

RESPONSES/COMMENTS (THE FUTURE OF PODIATRY) - PART 2C
From: Robert Kornfeld, DPM
I graduated from NYCPM in 1980 and opened my own private practice in 1982. And shockingly, there was no such thing as managed care. We functioned in an indemnity insurance model. You did your work. You sent a claim form (handwritten, there were no ICD-10 or CPT codes). You got paid 80%. The patient or secondary plans paid 20%. Bunionectomy with osteotomy back then through commercial carriers, for example, could reimburse in the $4-6,000 range.
On 20 patients per day, I ran a financially successful practice. I was happy. Until I wasn't. In the early '90s, if you weren't signed up with managed care, insurance paid much higher out-of-network fees. But little by little, they started drastically reducing those fees and made reimbursements more and more difficult to obtain. And I was losing patients to in-network podiatrists. So I signed up for...

RESPONSES/COMMENTS (MEDICAL ECONOMICS) - PART 1A
From: Bret M. Ribotsky, DPM
Dr. Kesselman’s recent commentary struck a chord because it echoes concerns I began writing about more than twenty years ago. At that time, I compared physician economics—particularly for podiatric physicians—to other professions, and argued that most doctors were focusing on gross income while ignoring delayed earnings, debt accumulation, overhead, and lost investment opportunity. Two decades later, the situation has not improved.
This is not an argument that teachers, transit workers, hotel employees, executives, nurses, or any other profession are over-compensated. Competitive markets should reward labor fairly. The question is why the physician—the individual assuming ultimate responsibility for diagnosis, treatment, outcomes, regulatory compliance, malpractice exposure, and years of specialized training—has become increasingly squeezed...

Hal was a truly kind, generous, loving, charitable soul. I am deeply saddened by his passing. The world is a lesser place for it.
Richard H. Mann, DPM
I was shocked and am very saddened by the recent passing of my friend, Dr. Hal Ornstein. He would be so thankful for and humbled by the thoughtful words of his friends and colleagues that have been posted on PM News and on Facebook. The posts concerning Hal were so well expressed and they were messages written from the heart about the type of person Hal was and the impact he had on others. I can tell you that he loved each of you.
He always appeared most satisfied when he was able to help anyone in need. As mentioned by those who’ve known and worked with Hal for years, he received many well-deserved accolades during his career. But what he appreciated most was simply the deep love shared with him by family and friends.
Hal and I first met at OCPM during...
