ID Podiatrist Opens New Office in Preston
Valley Foot & Ankle has announced that it will be expanding its services to Preston, Idaho, where it will provide residents with the same level of care given to the people in Pocatello. The owner, Brandon Wilde, DPM earned his podiatric medical degree at the Des Moines University, School of Podiatric Medicine and Surgery.
Dr. Brandon Wilde
Dr. Wilde completed a 3-year Podiatric Medicine and Surgery Residency based in Albuquerque, NM. He is Board Certified in Foot Surgery and Rearfoot/Reconstructive Ankle Surgery by the American Board of Foot and Ankle Surgery and is a Fellow of the American College of Foot and Ankle Surgeons.

PODIATRIC ECONOMICS
MD Podiatrist Explains the Math Behind 3-Year Physician Contracts
According to Mikel Daniels, DPM, president and chief medical officer of WeTreatFeet Podiatry, young job candidates are increasingly prioritizing lifestyle and job flexibility over partnership or long-term practice ownership. Modern applicants often seek fixed hours without nights, weekends, or call duties, marking a stark contrast to traditional patient-first mentalities where building equity in a practice was the ultimate goal. Dr. Daniels attributes this mindset shift largely to medical training environments, which are now predominantly led by hospital-employed physicians on productivity-based contracts rather than private practice attendings who instill an entrepreneurial, practice-building ethos.
Dr. Mikel Daniels
To account for this frequent turnover, WeTreatFeet structures its initial employment offers around a deliberate three-year financial framework. Under this model, the practice expects to lose money on a new doctor during their first year, hopes to break even in the second year, and finally turns a profit in the third year right before many physicians relocate or move on. Even with substantial student debt and guaranteed six-figure salary offers, modern candidates frequently choose these short-term stints for experience and location flexibility rather than investing in the long-term security of an established practice.

Podiatrists Among Most Likely to Look for an Out-of-State Job
Pathologists, hematologist-oncologists, and podiatrists are the most likely physicians to search for a job in another state, according to a Sept. 4 blog post from Marit Health. The analysis of job search behavior from 18,000 clinicians found wide variation in out-of-state interest by specialty, driven largely by how tight the local job market is. Pathology topped the list at 80%, followed by hematology, oncology, and podiatry at 78% each, dermatology at 75%, and cardiology and internal medicine tied at 71%.
Specialties Most Likely to Job Search Out of State
The pattern splits into two groups for different reasons, the blog post noted. Hematology oncology is a small specialty with strong demand, pushing physicians to search broadly for the right fit, while pathology and podiatry have large workforces chasing comparatively few openings, forcing a national search.

CODING CORNER
FY 2027 ICD-10 Changes That Will Bounce Your Claims on Oct. 1
Give Plantar Fasciitis Its Own Code, and a Side
Plantar fasciitis finally gets dedicated codes after 11 years of being billed under a code for a related but different condition. The new M67.A series splits it three ways: M67.A01 for the right foot, M67.A02 for the left, and M67.A09 when the record does not specify. The un-sided plantar fascial fibromatosis code gives way to M72.20, M72.21, and M72.22 on the same schedule.
The practical risk is not the code, it is the note. Physicians who have documented "plantar fasciitis" without a side for a decade will keep doing it out of habit, and every one of those notes now lands on an unspecified code that invites payer scrutiny and medical necessity denials. Podiatry, orthopedics, physical therapy, and primary care all need this in front of clinicians before October, not after the first denial report.

RE: Lack of Support for Used Swift Pro Devices
From: Lonny Nodelman, DPM
I've noticed that subscribers to your email periodically post their Swift machines for sale. I recently tried to sell my barely-used Swift Pro device. When I reached out to Emblation proactively, they told me they would offer no support to a new owner of a used device. That essentially renders the device worthless to resell, since you need an active account with them just to purchase the disposable tips it requires to function.
On top of that, Emblation appears to be phasing out the original Swift machine. Tip prices have already gone up, and there's a real possibility they'll stop offering tips for it altogether at some point. Meanwhile, I financed my machine through them and am still making loan payments — payments that don't stop just because the device might become unusable.

RESPONSES/COMMENTS (CLINICAL) - PART 1A
From: Gary S. Smith, DPM
I think Dr. Jacobs is really pushing these medications and am curious why. I think his whole argument comparing GLP-1 meds to NSAIDs is ridiculous. All I stated was that these meds need further testing before they are prescribed like candy by anyone. I work with ER docs who have told me that they are seeing people very often who are dealing with the side-effects of these medications; some of these side-effects are very serious. I only prescribe medications when the benefits outweigh the risks.

RE: Most Recent ADA Guidelines for Care of the Diabetic Foot (2026)
From: Allen M. Jacobs, DPM
There has been recent discussion in Podiatric Medical News regarding the potential role of GLP-1RA medications in the prevention and/or treatment of diabetic foot pathology. The following is taken from the most recent guidelines on care of the diabetic foot from the American Diabetes Association. I thought this may be of interest to those who evaluate and treat diabetic foot pathology.
With reference to peripheral vascular disease:
Emerging glucose-lowering therapies with PAD (and other cardiovascular disease) benefits include GLP-1 RA agents. Treatment with GLP-1 RAs may reduce risk of lower-extremity amputations, DFUs, and all-cause mortality compared with sodium-glucose cotransporter 2 (SGLT2) inhibitors (145). For example, oral semaglutide had a significantly lower rate of major adverse limb events (e.g., hospitalization for acute and chronic limb ischemia) in the designated cardiovascular outcome trial (146). In addition, the Semaglutide and Walking Capacity in People with Symptomatic Peripheral Artery Disease and Type 2 Diabetes (STRIDE) study investigated the impact of injectable semaglutide in individuals with type 2 diabetes and PAD who were identified at the earliest symptomatic stage of PAD (Fontaine stage IIa) (147). Semaglutide significantly improved minimal and pain-free walking distance, quality of life, and disease progression based on composite outcomes of rescue therapy initiation, all-cause death, or major adverse limb complications.
With reference to peripheral neuropathy:
Clinical evidence of potential benefits of GLP-1 RA agents on DPN is still controversial and limited (88). In contrast, altered skin sensation, including allodynia (i.e., pain evoked by contact, e.g., with socks, shoes, and bedclothes), has been described with use of either GLP-1 RA (89) or dual glucose-dependent insulinotropic polypeptide and GLP-1 RA agents (90) events within the 52-week study period. Post hoc subgroup analyses showed that the benefits of semaglutide were independent of baseline diabetes duration, A1C levels, BMI status, or concomitant SGLT2 inhibitor treatment.
