CA Podiatrist Jumps into Heated Supervisor Race
Fresno County Supervisor Nathan Magsig appears to be a shoo-in for a seat in the California Senate in the November general election. If he wins, it will free up the District 5 seat and trigger a special election. Now, another candidate is throwing his cowboy hat in the race. Allen Clyde, DPM announced his candidacy during a Tuesday appearance on KMJ’s Broeske & Musson Show.

Dr. Clyde is a retired podiatrist and longtime school board member. One of his top priorities is forest management. District 5 includes Clovis, parts of east Fresno, and the mountain communities.

Stop Assuming Every Thick, Yellow Nail Is Fungus
Key Points
- ~50% abnormal nails nonfungal; consider trauma, psoriasis, infection, aging.
- Thick yellow nail ≠ presumed onychomycosis; confirm before treatment.
- Nail clipping pathology or PCR can confirm fungal infection.
- If onychomycosis confirmed, set expectations: regrowth may take 18 months.
- Terbinafine pulse dosing: 1 week on/3 weeks off ≈ 4 months continuous.

Ways to Collect What's Due at the Time of Service
Front-end collections are rising while back-end recovery slides. These moves can get the balance paid while the patient is still there.
6. Give the front desk a script and a number to hit
Collections improve when somebody owns them. Practices reporting better patient balance collections in an MGMA Stat poll credited accountability for registration and billing staff as often as they credited any tool or vendor. Write the script, three sentences at most, and have every staffer use the same one: the balance, the request, the payment plan option.
Then track a single weekly number, the share of patient responsibility collected at or before the time of service, and post it where the team can see it. Benchmarks give the target. Copayments collected at the time of service ran around 56% in MGMA benchmarking data, well below the roughly 90% practices managed before the COVID-19 pandemic, which means the ceiling is higher than most front desks assume.

What Income is Not Taxed by the IRS?
It might feel as if the IRS taxes most of your hard-earned money, but some types of income are non-taxable.
3. Life insurance proceeds
Life insurance policy proceeds received by a beneficiary after the policyholder's death are generally tax-free. However, interest earned on the proceeds might be taxable, and tax rules can get complex if the policyholder surrenders the policy for cash.
If you take a life insurance policy loan, the loan generally isn't taxable if the policy remains in force and the loan amount doesn't exceed the amount of policy premiums paid.
The IRS has an online tool that can help determine whether life insurance policy proceeds you've received are taxable.

Lost in Translation: How Language Gaps Undermine Patient Care
Here are some valuable tips that have aided Savannah Santiago, DPM in navigating her experiences as a student and a resident:
5. With every patient — English-speaking or not — make it a point to ask them to repeat back their understanding of their condition and the next steps. Medical literacy varies widely, and most patients haven’t been trained in the language or systems of medicine like we have. They deserve clarity. They deserve to understand.
Something else I see far too often is healthcare providers attempting to speak broken or basic Spanish (or another language) during medical visits because it feels "faster" or "easier." But a patient encounter is not the time to practice your high school Spanish. It’s not about your language skills — it’s about the patient’s right to accurate, clear communication. I don’t care if you have a 100-day Duolingo streak — that patient has a legal right to a qualified interpreter, not to struggle through a conversation with someone who is barely conversational in their language.

Remember when you were a kid and you thought you could pull the wool over your parents’ eyes by not telling them the whole truth about your plans? You’d tell them the party was being chaperoned by Spencer Throckmorton’s uncle. Spencer Throckmorton’s uncle was only 19 years old, but you didn’t mention that fact. We hoped our parents didn’t ask for Spencer’s uncle’s CV.
Our three favorite podiatric guardians are pulling that same scam as I write this. You can’t believe how many people have asked me how many students are enrolled in the podiatry colleges. I’m the last person AACPM, CPME, APMA, or the colleges are going to confide in. Nothing has been published on the Internet. Saying nothing is only slightly more egregious than advertising each school has a 100% residency placement rate. J. Fred Muggs is a first-year resident this year. It keeps the federal funding flowing and it reinforces the truism that you can teach a monkey to...
