PODIATRIC QUICK POLL
How often do you take cultures for yellow toenails?
AT THE COLLEGES
NYCPM Welcomes Largest Incoming Class in Years
The New York College of Podiatric Medicine at Touro University (NYCPM) held its annual boat cruise around New York Harbor last week as part of new student orientation. With beautiful weather and a strong turnout, students had a chance to meet and get to know their classmates and new life-long friends.
NYCPM Incoming Class and Faculty
NYCPM faculty, administration, and student services staff also attended to give everyone a chance to meet and mingle in a relaxed and casual setting while enjoying the skylines of Manhattan, Brooklyn, Staten Island, and New Jersey, including the Statue of Liberty and Ellis Island. "We are pleased to welcome our largest class in years with 90 students, and look forward to their becoming a part of the NYCPM family," said Mike Trepal, DPM, Vice President for Academic Affairs and Dean.

PODIATRISTS IN THE COMMUNITY - PART 2
FL Podiatrist Volunteers on Medical Staff of 2026 National Scout Jamboree
Michael Ball, DPM of Boca Raton, Florida recently returned from the two-week National Scout Jamboree in West Virginia. He was the only podiatrist on the medical staff taking care of 15,000 participants. Both boys and girls, and adult staff attended and participated in high-intensity activities such as rock climbing, BMX racing, water sports, archery and shooting sports, and zip lining, while hiking an average of 7 miles daily in the hilly mountains of West Virginia. The National Scout Jamboree is held every four years.
Dr. Michael Ball
The medical staff was extremely busy treating injuries of every type. The Palm Beach County Podiatric Medical Association, Darco International, Amerx Healthcare, and Pedifix helped to sponsor the Jamboree with supplies and donations, which made it possible to effectively and efficiently take care of their patients so that most were able to return to some type of activity.

PRACTICE MANAGEMENT TIP OF THE DAY
9 Workflows Driving Your Claim Denials, Ranked
More than 400 revenue cycle leaders told Inovalon which workflows send their claims back. The ranking reads as a to-do list for whoever owns the front end of the practice.
No. 1. Insurance eligibility and benefits verification
Fifty-four percent named insurance eligibility and benefits verification, and 27% called it their single biggest driver of denials. Nothing else in the survey is close on either measure, and it holds across settings: 33% of ambulatory respondents, 28% of acute, and 18% of post-acute named it their top driver. Fifty-eight percent of respondents traced their single biggest source of denials to a front-end workflow, and eligibility is the largest piece of that. The uncomfortable operational implication is that eligibility is not a one-time check. Coverage lapses mid-episode. Patients switch plans and do not mention it. Secondary coverage gets missed.
Fifty-eight percent of respondents said they have responded by monitoring eligibility changes more closely rather than verifying once at patient registration, and at least one described re-checking eligibility every month for patients in active care. If you fix one workflow on this list, fix this one.

FINANCIAL TIP OF THE DAY
What Income is Not Taxed by the IRS?
It might feel as if the IRS taxes most of your hard-earned money, but some types of income are non-taxable.
5. Long-term care insurance income
Payments received from long-term care insurance policies are usually not subject to tax.
If you receive reimbursements for medical expenses due to injury or illness under an accident and health insurance contract, these payments are generally considered non-taxable by the IRS.

RESPONSES/COMMENTS (PODIATRIC STUDENT ENROLLMENT)
From: Rod Tomczak, DPM, MD, EdD
I want to thank Dr. Sherman for reminding us of the 2024 or 2025 metrics, Remember When? What you and Dr. Christina are missing is the matriculated students entering podiatry schools today, in 2026. We are worried about enrollment for a graduating class of 2030, which has not been posted as of the evening of August 16, 2026. We have been following enrollment for a couple of years now, have been posting about its decline, and are familiar with what AACPM posts and where they post it.
Is there an enrollment crisis, has class size been steadily decreasing, are we losing podiatry students to osteopathic schools, and are we burying our heads in the sand? Acceptance into podiatry schools used to be competitive, Dr. Sherman. Remember When?

RESPONSES/COMMENTS (PODIATRIC MEDICAL NEWS QUICK POLLS)
From: Bryan C. Markinson, DPM
I am not sure how people even answer the current survey question "How often do you take cultures for yellow toenails?" (although over 400 already have), without any context or nuance for which yellow toenails as a singular finding has tons! Those who responded NEVER are particularly curious to me. What about a patient with a history of skin disease? other autoimmune issues? other pedal skin issues? occupational issues? endocrine diseases? on chemotherapy? etc. Secondarily, why not ask about testing of any kind for yellow nails that may include culture, histology, and PCR?
After 40 years in this profession, do we still need the constant reminder that not everything is fungus? Do some clinicians still believe that "they know fungus when they see it?" Apparently so... but countless citations in the legitimate literature say they do not. My own estimate as to the accuracy of eyeball diagnosis by podiatric clinicians of toenails is probably around 85%. But a 15% error rate is not casual and wastes millions of dollars on treatments that should have never been started. Primary care docs and some dermatologists who empirically diagnose fungus also add to this problem of misdiagnosis, let alone all the current concern about antifungal stewardship.
Bryan C. Markinson, DPM, NY, NY

Editor's note:
We appreciate this feedback on the poll and will incorporate it into a future quick poll.

RESPONSES/COMMENTS (HEALTHCARE NEWS)
From: Ivar E. Roth, DPM, MPH
I totally respect Dr. Rubin's opinion, but this is the same dogma we have all heard forever. I am happy to explain the consequences of that thought process. I have treated thousands of patients in my career and have developed a cure for fungus toenails. Patients having been told they do not have fungus due to a lab test are hopelessly left with a problem they believe is not treatable or curable. This affects their self-respect as they have to hide their ugly nails and suffer the humiliation of this problem. Indeed, there is a cost to society for unnecessary lab tests and the patient as well for the co-pays, etc. So, if we know that the test is only correct half the time, what should a responsible doctor do? Tell the patient it is not fungus and not treat it? This is the real problem.
Using responsible pulse dosing on non-at-risk patients could be one answer. But orals at best only work 50% of the time anyway. Pulse dosing treatment requires no lab tests, is inexpensive, and safe. Topicals are entirely safe as well. Unfortunately, the medical profession does not believe that there is a cure, but I have spent 30 years working on a cure, and there is one. I do it every day, and those who follow directions will achieve at least a 95% cure rate. The 5% remaining may be indicated for potential lab tests. Using a shotgun approach to lab testing is a total waste of time and money.
