NY Podiatrist Performs TTT Limb-Saving Procedure

Huntington Hospital has performed its first transverse tibial transport (TTT) procedure, an advanced technique designed to stimulate new blood flow in patients with severe peripheral vascular disease who may otherwise face amputation. Jacqueline Prevete, DPM, a surgical podiatrist at Northwell’s Huntington Hospital, recently performed the procedure on a male patient. The patient was also under the care of vascular surgeon Dr. Sarrina Shraga.

“This is truly a limb salvage procedure for patients who may otherwise have no remaining options. Many of these individuals have already been through multiple vascular procedures and are still at risk for amputation. TTT gives us another tool to help restore blood flow and potentially preserve the limb,” Dr. Prevete said.

RESPONSES/COMMENTS (CODING CORNER)
RE: Oct 1 Plantar Fasciitis Code Change
From: Salman Lakhani

What every podiatry practice needs to do before October 1, 2026. On October 1, 2026, M72.2 — Plantar fascial fibromatosis — is deleted from ICD-10-CM. For years, this has been the code most podiatry practices report for plantar fasciitis. Two separate destinations replace it, and they are not interchangeable. For plantar fasciitis (a new code family): M67.A01 Plantar fasciitis, right foot M67.A02 Plantar fasciitis, left foot M67.A09 Plantar fasciitis, unspecified foot

For plantar fibromatosis/Ledderhose disease: M72.20 Plantar fascial fibromatosis, unspecified foot M72.21 Plantar fascial fibromatosis, right foot M72.22 Plantar fascial fibromatosis, left foot

Why This One is Likely to Slip Through

The obvious re-map is the wrong one. A practice updating its templates sees M72.2 disappear, finds M72.20 / M72.21 / M72.22 in its place, and maps straight across — landing every plantar fasciitis encounter on a...

RESPONSES/COMMENTS (INTERNATIONAL PODIATRY IN THE NEWS)
RE: Saudi Arabia Opens Diabetic Foot Center Focused on Limb Salvage
From: Abdulaziz Kimawi, MD, DPM

Dr. Sulaiman Al Habib Medical Group has opened a comprehensive Diabetic Foot Center of Excellence at Al Muhammadiyah Hospital in Jeddah, Saudi Arabia, with the primary goal of reducing preventable amputations through early intervention, coordinated multidisciplinary care, and advanced limb salvage. Diabetes affects approximately 23.1% of Saudi adults, while a Jeddah study reported lower-extremity amputation in nearly 30% of patients with diabetic foot ulcers, underscoring the need for dedicated diabetic foot programs.

Led by Dr. Abdulaziz Kimawi, MD, DPM, the center coordinates timely intervention and directs patients toward the appropriate specialty and treatment pathway. It integrates vascular surgery, endocrinology, infectious disease, wound care, internal medicine, emergency medicine, and other supporting specialties. The center provides comprehensive care including prevention, wound management, complex infection treatment, surgical offloading, Charcot reconstruction, revascularization coordination, and advanced limb-salvage surgery.

Dr. Kimawi graduated from King Abdulaziz University School of Medicine and later earned his DPM from Barry University School of Podiatric Medicine. He completed his podiatric surgical residency at Jackson Health System in Miami, followed by an ACFAS fellowship in reconstructive foot and ankle surgery at CHI Franciscan Health in Washington State. We welcome collaboration with podiatric physicians and limb-salvage programs internationally to advance diabetic foot care and amputation prevention.

RESPONSES/COMMENTS (GOVERNMENT SEQUESTER)
RE: Sequester Has Not Been Addressed
Gary S. Smith, DPM

Several years ago, the government could not pass a budget increase so they invented the Sequester to cut spending 10% across the board. This led to Medicare cutting doctors' pay by 10%. We were the only people affiliated with the government that got a cut in pay. In my opinion, this was illegal.

Medicare is required to pay 80% of the approved amount, period. They did force us all to renew our contracts the following year to allow them to rip us off. In 2017, the "sequester" was officially ended by the government yet Medicare continues to deduct it. When I call about this they tell us, "well, that's just what we have always done."

I can't help but wonder why the agencies like the PPMA and the APMA have not challenged this! I know some people are big supporters of these organizations but I have been charged over $60,000 so far during my career to be a member and all I've seen is our profession has not advanced and really important issues like this go unaddressed.

RESPONSES/COMMENTS (PODIATRISTS IN THE NEWS)
From: Ben Pearl, DPM

The description of the biomechanics gait adaptation with wide pants is perfect. Saying that the falls are not caused by wide silhouette pants (bellbottoms) itself is a funny thing to say. It is like Owen Wilson joking that it’s not the fire hydrant’s fault that his dog decides to use the facilities at that spot.
