PROPOSED MEDICARE CUTS
Proposed Modifier -25 Cuts Will Endanger Patients: OK Podiatrist
Western Oklahoma podiatrist Brad Nelson, DPM, said the insinuation that physicians are over-collecting for the healthcare services they provide Medicare patients is "insulting." Dr. Nelson has been running a private podiatry practice in Oklahoma since 2004, and he said reimbursement rates for Medicare and Medicaid have decreased significantly over the last couple of decades - by at least 33% for his practice, he estimated.
Dr. Brad Nelson
Nelson said about half of his patients rely on Medicare, and many are from rural areas. His office is based in Clinton and he has a satellite clinic in Elk City, drawing in patients from Altus, Hollis, Woodward, and parts of Texas. The proposed reimbursement cuts would particularly impact rural patients who have to drive long distances to get care, or patients who rely on others to drive them to an appointment, Nelson said. During a recent new patient visit, Nelson said he noticed a concerning spot and immediately conducted a biopsy. It came back as a Stage V melanoma. "Can you imagine him getting delayed treatment because Medicare wanted to pay us a 50% reduction?" Nelson said.

PODIATRISTS AND HUMANITARIAN EFFORTS
TPMA Mexico Mission Treats 750 Patients
The Texas Podiatric Medical Foundation (TPMA) recently treated 750 underserved and indigent patients at the Mexico mission. TPMA sponsored twelve podiatric residents to attend. The medical team volunteered for their 6 days of dedication to make a difference for these families at the 2026 Mexico Mission in San Miguel de Allende. 28 surgeries were performed and 500 orthotics were provided by Forward Motion and Redi-Thotics. Shoes were provided for seniors and children.
TPMA Mexico Mission Volunteers
Mission Chief Jim Kutchback, DPM, and Chris Browning, DPM, devote their time all year and want to thank the team for a terrific trip and their focus on making a difference in so many lives. This year, a memorial mural was painted in honor of Don Falknor, DPM, former Mission Chief and TPMA President at the San Miguel clinic.

QUERIES (CODING)
Query: E/M Coding: Office vs. Facility/Home Codes for Moderate-Complexity MDM
Does writing or managing a prescription with moderate-complexity medical decision-making (MDM) for an established patient in a podiatric office qualify for CPT code 99214? Furthermore, if this is correct, do CPT codes 99309 (nursing facility) and 99349 (home or residence visit) apply under the same MDM criteria, and why might those specific site-of-service codes be inapplicable depending on the setting or patient status?

RESPONSES/COMMENTS (CLINICAL) - PART 1
RE: Embolization as an Alternative Treatment for Chronic Plantar Fasciitis?
From: Allen M. Jacobs, DPM
Interventional radiologists and cardiologists have been advertising embolization as an alternative treatment for chronic plantar fasciitis. It appears to be founded on the theory that plantar fasciitis is an inflammatory disorder, thus reduction of inflammation by decreasing blood flow by embolization would reduce or eliminate heel pain. Obviously, the use of embolization is contrary to musculoskeletal literature, including the prevailing theories regarding the evaluation and treatment of plantar fasciitis in podiatric, orthopedic, and physical therapy/physiatry literature.
I have enclosed some references which I have been able to review suggesting that "neovascularization" is the cause of pain in plantar fasciitis. Those advertising the procedure claim partial weight-bearing in 2-3 days. They also note that the embolization may take 3-6 months to effect "significant pain relief". Gandhi, R., Banker, M. Br. J. Radiology 2024 Jan 27;97(1155): 544-548, Shibuya, M., et al. Cardiovasc Interevent Radiol 2021; 44(11): 1823-1826.

RESPONSES/COMMENTS (CLINICAL) PART 2
From: Gary S. Smith, DPM
So, to summarize all the research Dr. Jacobs cited, losing weight helps foot problems. That's great news. The question that remains is whether prescribing these meds is worth the potential side-effects. They DO happen and can be severe. Some people believe these drugs may be off the market in a few years. In the cancer studies on these meds, I was told that anybody that got cancer the first year of taking them was excluded from the study. Have you mentally taken a poll of your patients on these meds? How many have gotten cancer? Many lose their gallbladders and end up with pancreatitis. Some end up with complications from the gallbladder issues? Is it worth it? Do YOU want to be responsible?
