Gout Is Rising in Younger Adults: CA Podiatrist
According to Dana Figura, DPM, "Researchers analyzing data from the Global Burden of Disease Study found that gout cases in people between fifteen and thirty-nine years old rose substantially between 1990 and 2021, with incidence in that age group climbing faster than in the population as a whole. High body mass index was identified as the leading driver, alongside broader shifts in diet and lifestyle."
Dr. Dana Figura
"The part of this that matters most for you is where gout tends to show up first. The big toe joint, specifically the joint at its base, is the single most common site for a first gout flare. That is not a coincidence. It is one of the lowest points in the body; and it bears enormous load with every step, and uric acid crystals tend to settle in cooler joints, which the toe often is, compared to your core. Patients usually describe it as sudden, severe pain that can wake them from sleep, along with redness, swelling, and skin that is hot and tender enough that even a bedsheet touching it is unbearable," says Dr. Figura.

Heavy Hiking Packs are Destroying Your Feet: NY Podiatrist
Researchers who published a new study in the Journal of Functional Morphology and Kinesiology now say they have figured out how heavy a backpacking/hiking pack should be—no more than ten percent of your body weight. So, if you weigh 180 pounds, your pack should be 18 pounds max.
Dr. Gabriel Cooper
“Hikers should be mindful that the effects on their feet could be felt immediately, leading them to quit the hike due to the development of a blister or more long-term problems, such as the development of arthritis of one of the many joints in their feet,” Gabriel Cooper, DPM, a podiatrist at Icahn School of Medicine at Mount Sinai in New York City, who was not involved in the study, told Outside.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1
RE: Lack of Options
From: Rod Tomczak, DPM, MD, EdD
We have an end-of-summer block party in the driveway of one of the residents on our street. There are a surprising number of physicians who live on this block. One young woman is a pediatric neuro-oncologist who just retired from practice to take a job with a pharmaceutical company because she has had enough of babies dying of brain tumors. It just got to her. A level 1 ED doctor retired early and is now running a drug rehab facility. He wants to rehab the folks who have become frequent flyers to the ED and dependent on Narcan. Personally, he is burned-out with all the trauma and GSW that don’t make it. A bariatric surgeon is contented as division chair, and for those folks who need more than a GLP-1 to stay alive, he is happy to keep the oversize beds on his floor filled.
A young plastic surgeon fellowship-trained in burn surgery was promised division chair if he joined this hospital staff. The hospital staff reneged, and he joined a U.S. company that specializes in liposuction and micro-laser liposuction. A general surgeon takes all after-hours calls. Then there is a youngish pediatric gastroenterologist who is on permanent disability. Another ER physician retired early and volunteers to be on the medical school admissions committee.
Podiatrists do not have the luxury of entering a different specialty or practice another branch of medicine to earn an income and pay back loans. Perhaps that is one source of discontent in the profession and a reason to desire an MD or DO degree. If young podiatrists have not chosen wisely by going into podiatry and are unhappy in our profession, they are left with few other viable options to make a living, pay off loans, raise a family, etc.
Rod Tomczak, DPM, MD, EdD, Columbus, OH
RESPONSES/COMMENTS (NON-CLINICAL) -PART 2
From: Allen Jacobs, DPM
With reference to Dr. Tomczak’s discussion of podiatry as a vocation, I am reminded of the observations of Sir William Osler, “Happiness lies in the absorption of some vocation which satisfies the soul.” He furthermore advised his medical students, “Get early this relish, this clear, clean joyance at work, with which languor disappears, and all shadows of annoyance flee away.”
I have always attempted to impart the teachings of Sir William Osler upon the residents with whom I continue to enjoy working. His insights are as valuable today as they were over 100 years ago. In my experience, the late James Ganley, DPM, was the “Sir William Osler” of podiatry. Those who were fortunate to have been students of Dr. Ganley can appreciate the analogy. Both taught by parable. And both were great clinicians, scientists, educators, and philosophers.

RESPONSES/COMMENTS (RELEVANT RESEARCH)
From: Richard Boone, Esq
I have read the various comments about the role of podiatric physicians in prescribing GLP-1 medications to their patients. Although I am not a podiatric physician, I do have a certain amount of experience in assisting podiatrists with legal issues so I would like to volunteer an observation from a somewhat different vantage point. There is a huge difference between what you legally CAN do and what you SHOULD do in the best interest of the patient and yourself.
If you are the prescribing physician for a GLP-1 drug, then it becomes your responsibility to manage that patient during the patient’s use of the medication. Legally, that means you must follow the patient regularly and be able to recognize, diagnose, and treat the myriad complications and co-morbidities which are associated with or affected by the use of the drug. And you have to do that for as long as the patient is using the drug under your prescription even where the foot pathology for which the patient presented is long resolved. If you haven’t deduced where I’m going with this already, prescribing GLP-1 drugs for your podiatric medical patients dramatically increases your malpractice exposure.
If you have a patient whom you believe would be benefitted in the treatment of his or her foot pathology by the use of a GLP-1 medication, the smart way to do it, from the legal standpoint, is to refer the matter to the patient’s treating MD, or an appropriate specialist, for administration and management of the drug. You can, and should, then work in cooperation with the MD to ensure that the patient uses the drug safely and effectively, and gets the required care without creating some future legal problems for yourself.
