MEETING NEWS
ISFRI Founders Present at University of Notre Dame Orthopedic Society
Karli Richards, DPM, MHA, and Amol Saxena, DPM, MPH, along with fellow International Society for Footwear Research & Innovation (ISFRI) founding board member Adam Tenforde, MD, presented at this past weekend’s meeting in South Bend, IN.
(L-R) Drs. Robert Richards, Karli Richards, Amol Saxena, Fred Ferlic, and Tenforde, along with Tayco Brace CEO Gavin Ferlic.
Both Robert Richards, Sr., MD, along with Seminar and Tayco Brace founder, Fred Ferlic, MD, are orthopedic surgeons. The meeting originated as a tailgate for Notre Dame alumni who became orthopedists, but Dr. Ferlic expanded to include more specialties, including podiatrists. Alumna Dr. Karli Richards spoke on a panel, “Fighting for the Female Athlete: Closing the Gap in Research, Performance, and Injury Prevention”, while Drs. Tenforde and Saxena spoke on shockwave.

QUERIES (CODING & BILLING)
Query: Anthem Orthotic Recoupment
I recently received multiple remittance notices showing payment reversals on orthotic claims going back to 2024.
Prior to submitting these claims to Anthem, we did our due diligence by calling and documenting their authorization for the orthotics. One such example was on a claim submitted 22 months ago. Anthem cited that the patient's benefit maximum has been reached. They then put the fee as patient responsibility.
This raises some serious questions:
1. How can a claim be approved and paid if the benefit maximum has already been exhausted?
2. Shouldn't insurance companies be required to provide a clear explanation and supporting benefit history before recovering payments that were previously approved?
3. If the insurer originally paid the claim in full and subsequently reverses that payment, is the provider legally permitted to bill the patient?
4. Are there contractual or statutory limitations on how far back Anthem may recover payments from providers in New York?
5. What protections exist for participating providers when insurers retroactively change their original payment determinations?
I am particularly concerned about the impact on patients who may unexpectedly receive bills for services provided nearly 2 years earlier, despite having been told at the time that the insurance company covered the services. Furthermore, which of these then fall under surprise billing?

RESPONSES/COMMENTS (ARTICLES)
RE: Political Advocacy and Podiatry: Does it Really Help?
From: Sev Hrywnak, DPM, MD
Political advocacy can sometimes benefit medical professional groups, but there are several ways it may fail to help podiatry in practice, especially when the advocacy goals are broad, the policymaking process is slow, or the outcomes are not directly tied to patient care, reimbursement, or day-to-day clinical capacity.
1. It often produces “policy wins” that don’t translate into care on the ground. Health systems and reimbursement rules usually change slowly, so even successful advocacy may not improve access in the short term. Many policy outcomes are administrative (paperwork, reporting, definitions) rather than clinical (fewer barriers, better referral pathways, faster appointments). Podiatry’s biggest constraints are frequently operational, workforce distribution, appointment capacity, orthotics/lab workflows, imaging referrals, and payer authorization, areas where legislation may not move the needle.
2. Advocacy can dilute podiatry’s most actionable levers (payment + access + scope). Podiatry’s value proposition to patients and payers is often best advanced through reimbursement specifics (codes, coverage criteria, medical necessity standards), not...
