PRACTICE MANAGEMENT TIP OF THE DAY
9 Ways to Tighten Patient Collections at Your Practice
Patients are increasingly primary payers. Here are nine front-end moves to keep balances from sliding into bad debt.
9. Track the metrics that actually drive change
Total patient collections is a lagging indicator. The metrics that predict where the number is going are leading ones: point-of-service collection rate, percentage of visits with a current estimate on file, percentage of self-pay statements paid within 30 days, average days in AR for patient-responsibility balances and the rate at which aged accounts roll into bad debt.
Pick three to five that map to your workflow, build them into a weekly dashboard and review them with the same seriousness as clinical quality measures. Practices that manage by total collections always look surprised by the number. Practices that manage by leading indicators see the number coming.

Grading Yourself After Surgery
Here are some valuable tips that have aided Savannah Santiago, DPM in navigating her experiences as a student and a resident:
6A. A Few Rules for the Road
Don't grade yourself immediately after a case. The emotional intensity, adrenaline, and mental fatigue can distort perspective. Give yourself space before reflecting.
Be honest without being unkind. Residency is a learning process by definition. A lower grade in one domain doesn't erase strengths in another. The goal is not a single flat judgment — it's a nuanced picture of your full performance.
Share the process with someone. A mentor, a trusted colleague, a program director. An external perspective keeps your internal evaluation grounded. Without it, self-criticism or self-doubt can quietly distort your perception over time.

CODING & BILLING Q&As FROM CODINGHELPLINE.COM
Query: Prior Authorization for Bunionectomy with Osteotomy?
For bunionectomy procedures involving an osteotomy, what is the documentation required to usually pass and get prior authorization?
Codinghelpline com Subscriber
Response from Codinghelpline.com: When the decision to perform a bunionectomy procedure involves an osteotomy, the insurance will most likely expect to see an intermetatarsal angle greater than 9. Up to 9 is normal in their per reviewed literature, so anything greater should suffice. Also, other radiographic findings should be in your radiology report or discussion about performing the specific procedure(s) planned. Avoid generic educational paragraphs. If educational information is included, it should be edited to reflect the specific patient's condition, conservative treatment, response to treatment, and clinical rationale:
Document failed conservative care. Clearly explain: what conservative treatments were attempted, how long they were utilized, why they failed, and why surgical intervention is now medically appropriate.

RE: 2027 Physician Fee Schedule Is a Threat to Every Podiatric Practice — and Our Silence Is the Bigger Danger
From: Eric J. Lullove, DPM
I am writing to sound an alarm that too few of our colleagues seem to be hearing. On July 14, 2026, CMS released the CY 2027 Physician Fee Schedule proposed rule (CMS-1848-P), and buried within it are provisions that, if finalized, will strike podiatry harder than almost any other specialty. CMS is proposing to reduce payment when a physician bills a separately identifiable office or outpatient evaluation and management visit on the same day as a 0-, 10-, or 90-day global procedure. In practical terms, the most expensive service — either the surgical procedure or the E/M visit — would be paid at 100 percent, and every other procedure or E/M visit furnished on the same day would be paid at 50 percent.
Read that again. A 50 percent cut on the second service, every time a patient presents with more than one problem on the same day.
This is not a theoretical concern for some distant subspecialty. CMS itself expects this proposal to have a large negative impact on otolaryngology, dermatology, podiatry, and...

RESPONSES/COMMENTS (PODIATRIC JOURNALS)
RE: Foot and Ankle Journals
From: Rod Tomczak, DPM, MD, EdD
The July/August issue of The Journal of Foot and Ankle Surgery, (JFAS) published by ACFAS came out last week. There were 34 peer-reviewed articles. Foot & Ankle International, published by the American Orthopedic Foot and Ankle Society contains 18 peer-reviewed articles. In contrast, the Journal of Bone and Joint Surgery (JBJS), the mother of all orthopedic journals, featured 20 scholarly articles.
Naohiro Shibuya, DPM, MS is to be congratulated for editing what appears to be the eminent foot and ankle journal based on the number of articles per issue. The rest of ACFAS, however, deserve a big “shame on you” and should beware of dislocating their shoulders patting themselves on the back. ACFAS is not in the least prolific. In the recent JFAS, of the 34 peer-reviewed articles, eight, a soft 8, not a hard 8, were written by...

RESPONSES/COMMENTS (EDITOR'S NOTE) - PART 1A
From: Alan Sherman, DPM

Our editors provide a very valuable service for us - they sift through the news, the opinions, the facts and disinformation, and give us their valuable opinions as to what we should pay attention to. We become not only readers of their publications, but fans of them when, over the years, we learn to trust their opinions. It's not as simple as that - sometimes we need to push back on them as we know their personalities, their enthusiasm, when they being realistic and when they are being visionaries. Barry Block has been all of that for all of us for all of these years. When he puts out an editorial, I always read it, because he is almost always a visionary worth reading, so long has he had his finger on the state of podiatry.

Frankly, I am excited to see Podiatric Medical News Issue #8,326 this morning because I believe that Barry has a lot more to say in his editorials, and a lot more to teach us in his choice of content. Best of luck to Barry and his family in carrying on this long-established tradition, Barry! I will always be a fan. And one additional note - diversity of content is good for us as a profession. I am also looking forward to seeing the editorial work that Kane Communications, Inc. puts forth and particularly, whom they choose to teach us. Long live the healthy flow of information in podiatry!
