Deadly Drug-Resistant Fungus Is Now in 23 U.S. States
A little-known fungus is quietly racing across the U.S., and doctors say the tools to stop it are thin. Candida auris, first spotted in the country in 2013, has now been confirmed in 23 states and is spreading fast, especially in hospitals, per CDC data cited by People. The agency logged 3,348 cases last year; as of July 18, this year's tally has already reached 3,130, putting 2026 on pace to set a record.
The bigger problem: Strains often rebuff standard antifungal drugs. "Many strains are resistant to azoles [a major antifungal drug class] ... and some are also resistant to amphotericin B, which is often considered a drug of last resort," says UT Health's Nathan Wiederhold. Texas currently leads this year's tally with 706 cases, followed by Michigan at 503. "The public should definitely not panic but should be concerned," Dr. Peter Chin-Hong of UC San Francisco tells Healthline.

RELEVANT RESEARCH
New Publication Highlights COVID-19-Associated Peripheral Neuropathy
Richard H. Mann, DPM and Vanessa Nieves, DPM have published a new peer-reviewed paper, "Resolution of Peripheral Neuropathy Associated with Acute COVID-19 Infection Following Benfotiamine and Methylcobalamin Supplementation: A Case Report," in Medical Research Archives, the official journal of the European Society of Medicine.
Drs. Richard Mann and Vanessa Nieves
The paper presents a case of severe, treatment-resistant peripheral neuropathy that developed during acute COVID-19 infection and resolved following adjunctive supplementation with benfotiamine and methylcobalamin. In addition to the case report, the authors review the growing body of evidence linking COVID-19 to peripheral neuropathy through mechanisms that may include inflammation, oxidative stress, mitochondrial dysfunction, and impaired thiamine metabolism. The authors conclude that these findings warrant further investigation in prospective controlled clinical trials.
PODIATRISTS AND LIMB CARE
CA Podiatrists Meet with Rep to Discuss Limb Preservation Care
California podiatrists David G. Armstrong, DPM, MD, PhD, and Wenjay Sung, DPM, met with U.S. Rep. Judy Chu (D-CA) at her Pasadena district office to discuss podiatry's role in preventing avoidable lower-extremity amputations in people with diabetes. The discussion focused on improving access to therapeutic footwear, regular foot screening, early treatment of wounds, and coordinated limb-preservation care.
(L-R) Drs. David Armstrong and Wenjay Sung; U.S. Rep. Judy Chu and case worker Amaiya Rucker
Dr. Armstrong directs the USC Limb Preservation Program and the Southwestern Academic Limb Salvage Alliance. Sung is a renowned Pasadena podiatrist and constituent of Rep. Chu. The meeting was coordinated with support from the American Podiatric Medical Association and emphasized that efforts to eliminate preventable amputation often begin with something deceptively simple: the right shoes and timely podiatric care.

RESPONSES/COMMENTS (NON-CLINICAL)
RE: Why Every Podiatric Practice Needs a Continuity Plan
From: Bret Ribotsky, DPM
I think every practicing podiatrist needs to sit with this: none of us are immune to sudden, unplanned disruption. I raise this not as an abstraction. I’ve lived it. I’ve had a key employee walk out the door with no notice — someone whose institutional knowledge, patient rapport, and daily operations I’d built real dependence on, gone in an afternoon. I’ve also experienced an acute, disabling injury that instantly took me out of clinical practice, with zero runway to prepare a transition plan, cross-train staff, or line up coverage. Both events forced me to learn, in real time and under pressure, how thin the margin is between a well-run practice and a crisis.
I doubt I’m unique. Every practitioner is one resignation letter, one burst pipe, one fire, one vendor bankruptcy, or one bad diagnosis away from a similar reckoning. Yet how many of us have an actual written plan for it? Who covers your patients if you’re suddenly unable to work? Do you have a cross-trained backup for your most critical staff role? Do you know your alternate vendors before you need them, not after? Is there a continuity plan sitting in a drawer somewhere, or does it exist only in your head — which is precisely the asset most at risk in a crisis?
Abrupt decisions can happen that leave people scrambling. Our practices are no different. I’d encourage every reader to actually build — and periodically test — a crisis and continuity plan for their own practice, before the crisis picks the timing for you.
Bret Ribotsky, DPM, Fort Lauderdale, FL
RESPONSES/COMMENTS (MEDICARE NEWS) -PART 1A
From: Allen Jacobs, DPM
I shall loosely refer to as "nail care" as a needed office visit for services unrelated to the nail care, typically onychomycosis. Do not qualify the patient for Medicare nail coverage. Therefore, you may require patient payment for any requested nail care. Increase your knowledge in areas such as neurology, dermatology, vascular disease, practical biomechanics. Treat more pathology and generate level 3 visits. Do more in-office testing. Treat onychomycosis.
I suggest that many patients who "qualify" for Medicare nail coverage would not actually qualify if the patients' examinations were compared to the documentation submitted. Remember, if not a Medicare covered service, you may allow an assistant to provide the nail care while you attend to more important matters, such as devoting more time to and generating more income with other patients.
Maybe it is time to give up Medicare nail coverage. It is insulting as well as degrading to confabulate class findings to be paid less than an immigrant pedicurist for the same or similar service. How much is your time worth? If it is that important, the patient should be informed of the truth. Medicare does not cover that (nails). Charge what you believe is an appropriate fee for your valuable time. Or, as others have suggested, convert to a direct pay practice.
4 years plus a 3-year residency for toenails and the need for near fraud to be paid little? It is time to re-define primary care podiatry as other than mostly toenails. If people want toenail care, they are the ones who have 2 choices. They can pay you or pay for a pedicure.

OBITUARIES
RE: The Passing of Paul Edward Tipton, DPM
Paul Edward Tipton, born in Hickman, Kentucky, built a life centered on family, friendship, and caring for others. Dr. Tipton earned his podiatric medical degree from the Illinois College of Podiatric Medicine in Chicago. In 1976, Paul and his wife Anne returned to Kentucky, where they established their home and Paul founded a podiatric practice that became one of the state's most recognized and admired.
Dr. Paul Tipton
Paul's commitment to his profession extended beyond his own practice. He served as President of the Kentucky State Board of Podiatry and represented Kentucky as a Delegate to the American Podiatric Medical Association's House of Delegates. As one of the first attending surgeons in Kentucky's inaugural Podiatric Surgical Residency Program, he helped train many of its early surgical residents, leaving a lasting influence on the physicians who followed.

RESPONSES/COMMENTS (OBITUARIES)

RE: The Passing of Arthur Erwin Helfand, DPM

The podiatric profession has lost one of its absolute giants. Arthur E. Helfand, DPM passed away at the age of 91, leaving behind a profound legacy that reshaped podiatric public health, podogeriatrics, and diabetic foot care. For the medical community, he was a tireless educator and visionary advocate. For me, he was the reason I became a podiatrist.
Long before podiatric medicine achieved recognition in mainstream medicine, Dr. Helfand recognized that foot health was linked to overall systemic longevity, mobility, and independence. Throughout his clinical career and decades as Professor at the Pennsylvania College of Podiatric Medicine (now Temple University School of Podiatric Medicine), he championed early intervention to prevent severe complications, particularly amputations in vulnerable diabetic populations. As such, he served to bridge the gap between podiatry and mainstream medicine, bringing the fields closer together for the common good better patient care. He served as a consultant to the US Department of...
