CA Podiatrist Provides Advice on Treating Dry, Cracked Feet of Summer
"Small daily habits can go a long way toward preventing painful cracked heels and keeping feet healthy all summer long," explains Nasim Kalhor, DPM, a board-certified podiatrist. To treat dry, cracked skin, you'll want to exfoliate and moisturize regularly using the right tools. "I recommend gently exfoliating thickened skin and moisturizing daily with ingredients such as urea or lactic acid to keep the skin soft and healthy," says Dr. Kalhor.
Dr. Nasim Kalhor
Once you've exfoliated, it's time to treat your toes with moisturizer. "I recommend products that both soften thickened skin and help maintain healthy feet," says Dr. Kalhor. "Moisturizers containing urea or lactic acid are excellent for reducing calluses and improving dry, cracked skin." If you're taking steps to treat your dry, cracked skin, it's equally important to address the root cause of the problem— otherwise, you'll end up in the same situation in the future. "Treating the underlying cause instead of simply masking the symptoms is the best way to restore healthy skin," explains Dr. Kalhor. Bottom line? Ditch the unsupportive flip-flops and get a pair of ergonomic summer sandals instead.

PRACTICE MANAGEMENT TIP OF THE DAY
Ways to Collect What's Due at the Time of Service
Front-end collections are rising while back-end recovery slides. These moves can get the balance paid while the patient is still there.
1. Ask for a card on file when you book the appointment
Card on file turns a collections problem into an authorization problem, and it moves the awkward conversation off the front desk and into the scheduling call, where staff are already talking about money. More than nine in 10 organizations surveyed by PayZen and HFMA now encourage payment, require payment, or collect a payment method during the estimate process, up from roughly eight in 10 the year before.
The mechanics matter. Disclose the maximum amount you will run without a second authorization, put the terms in writing, and confirm the balance by text before charging anything. Practices that skip the disclosure step generate the complaints that get card-on-file programs killed. Practices that handle it plainly tend to find patients prefer it to another statement cycle.

SOLE PURPOSE BY SAVANNAH SANTIAGO, DPM
Grading Yourself After Surgery
Here are some valuable tips that have aided Savannah Santiago, DPM in navigating her experiences as a student and a resident:
Self-assessment can either accelerate growth or fuel imposter syndrome, depending on how you approach it. It's useful to break evaluation down to 4 domains.
6B. A Few Rules for the Road
Revisit similar cases across your training. Growth is hard to perceive day to day, but obvious when you compare early cases to later ones. The procedure that once felt overwhelming gradually becomes more familiar and more controlled. Seeing that progression directly is one of the strongest antidotes to imposter syndrome.
Finally — outcomes and surgical quality are not always perfectly aligned. A good outcome does not mean a perfect operation, and a complication does not automatically mean poor judgment. The most meaningful evaluations focus on the quality of your decisions and execution, not outcome alone.

RESPONSES/COMMENTS (THE FUTURE OF PODIATRY)
RE: Podiatry is Sitting on a Gold Mine
From: Bob Hatcher, DPM
I think retired podiatrists could be a gold mine of wisdom, volunteer help, and funding for the profession if someone had the energy to organize them.
Bob Hatcher, DPM, Raleigh, NC
RESPONSES/COMMENTS (CODINGHELPLINE.COM)
From: David Secord, DPM

With this radiographic explanation as documented in my normal charting, I don't believe I've ever had a denial.

Radiographic Evaluation: Bunion pre-op radiograph

On pre-operative radiographs, the patient has the following clinical findings:

An 18 degrees IM (intermetatarsal angle) on the right AP view and an IM (intermetatarsal angle) of 18 degrees on the left AP view (normal is 9º). The talocalcaneal angle (Kite’s angle) is  to the 1st ray, bilateral. The patient also has a  level of metadductus (22 degrees, with normal 15 degrees), bilateral. The HAV (hallux abducto valgus) angle is 24 degrees (normal is 15 degrees), bilateral. The HAI (hallux abducto interphalangeus) angle is 10 degrees (normal is 0 degrees-5 degrees) on the right foot and 10 degrees (normal is 0-5 degrees) on the left. The sesamoid position on the right is 5/7 (normal is 1). The sesamoid position of the left AP view is 5/7. PASA angle measures 10 degrees (normal is 8 degrees)  bilateral, and the DASA angle measures 10 degrees (normal is 8 degrees). The angle of the articulation of the base of the 1st metatarsal to the medial cuneiform is___, and ___.

The lateral views show a calcaneal inclination angle of 25 degrees (normal is 35-40 degrees). The talus is  below the 1st ray on the lateral view, bilateral. The joint surfaces show articular cartilage with only the 1st MTPJ (metatarsal/phalangeal joint) showing any exostoses and without dorsal spurring. These findings are consistent with HAV (hallux abducto valgus), bunion deformity and HAI (hallux abducto interphalangeus) deformity of the right and left feet.

RESPONSES/COMMENTS (MEDICARE)
From: Ivar E. Roth, DPM, MPH
I would like to provide a private concierge direct payment view of what Dr. Lullove has expressed. While I agree that Medicare and other insurance companies are screwing our podiatric profession, my view is different. As a medical chart reviewer of many years, I can say that a segment of our profession way over utilizes and abuses the insurance carriers. I can also say that when a private patient comes into my office and I have to bill them for the services which they pay for before they leave, I bundle and include all services within reason into 3 categories, either a simple office visit, intermediate, or extended. They are based on time I actually spend with the patient, not on how many services I provide. Of course, x-rays and orthotics and those types of things are extra. The vast majority of my services probably 70% are in the simple category, 20-25 percent are intermediate, and 5 to 10% are extended.
The vast majority of my patients are extremely appreciative of the services I provide and are patients for life. I give them great...
Editor's note: Dr. Roth's extended-length letter appears here.
