PODIATRISTS IN THE NEWS
Feet are Often First Indicator of Systemic Conditions: NY Podiatrist
“Feet are the farthest body parts from our heart and our lungs, so they’re the last to get the message — the oxygen, the nutrients, the growth factors,” said Miguel Cunha, DPM, a podiatrist and founder of Gotham Footcare. Several body systems have to be operating smoothly to get blood and nerve signals all the way down and back up, Dr. Cunha said, which means that when a systemic issue occurs, feet can be the first to reveal it.
Dr. Miguel Cunha
Diabetic foot ulcers typically occur on the soles of the feet or between the toes, where pressure or friction causes the injury, and then poor circulation and immune function keep it from healing. You also might not initially notice the ulcer, because of neuropathy, which can make it worse, Dr. Cunha said.

PRACTICE MANAGEMENT TIP OF THE DAY
Ways to Collect What's Due at the Time of Service
Front-end collections are rising while back-end recovery slides. These moves can get the balance paid while the patient is still there.
2. Make the payment ask private
The biggest obstacle to time-of-service collection is that the ask happens at a counter with three people standing behind the patient. Nobody wants to negotiate a $600 deductible in public, so the patient says they will pay later, and the staffer, who also does not want the conversation, lets it go.
Moving the prompt to a tablet, a kiosk or the patient's own phone at check-in removes the audience. It also removes the judgment call from the front desk, because the system asks every patient the same way, every time. If a kiosk is not in the budget, a smaller change works: Hand the patient a tablet turned toward them, step back and let them read the balance before anyone says a word about it.

FINANCIAL TIP OF THE DAY
What Income is Not Taxed by the IRS?
It might feel as if the IRS taxes most of your hard-earned money, but some types of income are non-taxable.
1A. Financial gifts
Financial gifts are a well-known category of non-taxable income. That's due in part to the generous annual federal gift tax exclusion. For example, you can give up to the limit to friends, family or anyone else and not be taxed. (The recipients won't be taxed on that amount, either.)
The tax limit for gifts given in 2026 remains at $19,000, $38,000 for married couples. This is one of many IRS provisions adjusted annually for inflation, so you can give up to that amount to as many people as you want without incurring tax liability. Note: Staying under these limits per recipient exempts you from filing a gift tax return for the year. But exceeding the limit doesn't necessarily result in owing tax, thanks to a high lifetime estate and gift tax exemption.

SOLE PURPOSE BY SAVANNAH SANTIAGO, DPM
Lost in Translation: How Language Gaps Undermine Patient Care
Here are some valuable tips that have aided Savannah Santiago, DPM in navigating her experiences as a student and a resident:
1. Effective communication is the bedrock of safe, compassionate, and equitable healthcare. Yet for millions of patients across the United States, that foundation is compromised simply because they do not speak English or have a low proficiency in English. In a country as linguistically diverse as ours — where one in four patients comes from a non-English-speaking household — language barriers are not a fringe issue. They are a central challenge that directly affects health outcomes, patient safety, and trust in the medical system.
As medical providers, we spend years learning to diagnose, treat, and manage disease. But none of that knowledge can be meaningfully applied if our patients don't understand us — or worse, if we mistake politeness or confusion for comprehension. When language becomes a barrier, miscommunication can lead to delayed care, inappropriate treatment, unnecessary suffering, and even death.

RESPONSES/COMMENTS (CODINGHELPLINE.COM)
From: Jeff Pinsky, DPM
Great x-ray notes by Dr. Secord. If I were the reviewer on this case for the insurance company, my first thought would be it’s too long didn’t read. Secondly, I’d take all the norms in parentheses as a key that the document is trying to lead my thinking. I’ve got my own set of norms defined by our medical team. Lastly, my denial would be on the grounds that we do not cover cosmetic procedures.
Just stating in the notes that the patient has skewed foot bone positions and angles without mention of the degree of pain and disability, and when these occur, would be our rationale for denial. The reviewer's job is to deny claims for service or pre-authorizations that don’t clearly state the key words my medical team has laid out for my job. I’m glad I don’t work for the insurance company.

RESPONSES/COMMENTS (THE FUTURE OF PODIATRY) - PART 1A
From: Jim Mothershed, DPM
I agree with Bob Hatcher’s comment on retired podiatrists being a gold mine of wisdom and judgment for our profession, which is not currently being utilized.

RESPONSES/COMMENTS (THE FUTURE OF PODIATRY) - PART 1B
From: Howard Zlotoff, DPM
Dr. Hatcher touches on a golden opportunity that has been overlooked for too long. Retired podiatrists across the country have experiences and expertise that are just waiting to be tapped.
The past several months of opinions about the state of our profession makes me realize that a large swath of the younger population, especially the pre-med students in high school and college, don’t even know what podiatry is! This is reflected in the low application rate to our schools. Our national organizations have the ability to organize and manage a program where active and retired podiatrists across America could offer presentations to the pre-med students societies on campuses and at career day events at high schools. A simple outline with some interesting pictures, x-rays, surgical spots, sports medicine pictures, etc. can be created. This would serve as a skeleton for the speakers. It would need to be professionally-created to impress the audience and help the speaker give a great talk.
The vast majority of colleges have a pre-med program hungry for presentations to their members. I had the opportunity to do these talks at a local college and several high schools and those students that showed interest were offered the opportunity to shadow our podiatrists at the office to see for themselves what a vast field podiatry really is. One can pursue general practice, surgery, sports medicine, research, pediatric care, etc. It’s a unique profession for sure. Let’s use every tool in the toolbox to enhance our profession. Retired doctors are a great place to start.
