IL Podiatrist Discusses Gout
Gout attacks, which most commonly affect the joint of your big toe, are sudden, excruciating, and can wreak havoc on your everyday routine. The pain and swelling in your foot can be severe — so severe, in fact, that wearing shoes may not even be possible at some points: "At the height of the pain and swelling, some patients can't even tolerate wearing socks, let alone shoes," says podiatrist Stephanie Wu, DPM, dean of the Dr. William M. Scholl College of Podiatric Medicine at Rosalind Franklin University of Medicine and Science.
Dr. Stephanie Wu
To make it even worse, gout tends to come back. According to Dr. Wu, studies have found that 60% of patients who suffer a gout attack will experience a recurrent flare within a year. While avoiding footwear during the worst of an acute attack may be the best policy, it's a good idea to find shoes that won't aggravate your healing joints after it subsides.

PRACTICE MANAGEMENT TIP OF THE DAY
Ways to Collect What's Due at the Time of Service
Front-end collections are rising while back-end recovery slides. These moves can get the balance paid while the patient is still there.
3. Build payment plans patients can finish
A payment plan a patient abandons in month three is worse than no plan, because it delays the moment anyone notices the account is not going to pay. Size the plan to the balance instead of applying one term to every account, automate the recurring charge so nobody has to place a call and set the minimum where the patient can keep up.
Flexibility is worth real money. Six in 10 patients told Experian Health they would feel more confident in their ability to pay if they were offered a plan that accounted for their financial situation. Watch the concentration, too. The share of patient collections tied up in open payment plans fell from 30% to 23% as front-end collection improved, which is the direction practices want.

FINANCIAL TIP OF THE DAY
What Income is Not Taxed by the IRS?
It might feel as if the IRS taxes most of your hard-earned money, but some types of income are non-taxable.
1b. Financial gifts
  • Charitable gifts are generally non-taxable. Be sure to get receipts and ensure the charities you give to are legitimate.
  • Be aware of key changes to charitable giving tax rules for 2026.
  • Unfortunately, gifts given by employers to employees that are akin to cash, i.e., gift cards, are usually considered taxable by the IRS.

Lost in Translation: How Language Gaps Undermine Patient Care
Here are some valuable tips that have aided Savannah Santiago, DPM in navigating her experiences as a student and a resident:
2. Interpreter services are often underutilized or dismissed as an inconvenience. This article is a call to pause — to reflect on how we, as healthcare professionals, show up for patients with limited English proficiency. It’s about the power of language, the legal and ethical responsibilities we carry, and the human stories behind every nod that hides confusion, behind every smile that masks fear. It is about why interpreter services are not optional — they’re essential.
Studies have shown that the use of professional translation services is associated with better postoperative outcomes, lower infection rates, improved management of chronic diseases, reduced blood pressure, and even lower cancer mortality rates. The benefits go beyond clinical outcomes. Non-English-speaking patients who receive interpreter services experience shorter hospital stays, report greater trust and confidence in their healthcare providers, and make fewer emergency department visits — all contributing to lower rates of readmission. In short, using a translator leads to better outcomes, better care, and healthier patients. It may be inconvenient, yes — but it is absolutely worth it.

CODING & BILLING Q&As FROM CODINGHELPLINE.COM
Query: Rules Regarding L3000 Usage in A5500
I was always taught that a diabetic insert (A5512/13/14) must be worn in a diabetic shoe (A5500). But in some circumstances patients just need more support than what A5514 can provide. Is it okay to get patient diabetic shoes (A5500) that they qualify for (without diabetic insert) and L3000 which they also have coverage and qualify for? If so, am I able to document recommending them wearing those non-diabetic inserts in their diabetic shoes?
Codinghelpline.com Subscriber
Response from Codinghelpline.com: Here is what the Therapeutic Shoe LCD and PA in part states: "Separate inserts may be covered and dispensed independently of diabetic shoes if the supplier of the shoes verifies in writing that the beneficiary has appropriate footwear into which the insert can be placed. This footwear must meet the definitions found in this policy for depth shoes or custom-molded shoes."
It further states: One pair of depth shoes (A5500) and 3 pairs of inserts (A5512, A5513, or A5514) (not including the non-customized removable inserts provided with such shoes).
A modification of a custom molded or depth shoe may be covered as a substitute for...

RESPONSES/COMMENTS (THE FUTURE OF PODIATRY)
From: Burton J. Katzen, DPM
I agree with my good friend Dr. Zlotoff and others that we all must do more to promote our great profession, but I think the major problems with enrollment fall elsewhere including:
1. The reimbursements for our services, especially surgery.
2. The number of years that it takes to become a podiatrist compared to other medical service professions, especially those such as CRNAs, optometrists, and PAs, many of which are ahead of us in average yearly earnings.
3. The fact that most states require a 3-year surgical residency whereas many graduates not only do not possess the necessary skills to do so but actually are more interested in practicing the many other aspects that drew many of us to the profession in the first place.
