CA Podiatrist Discusses Lymphatic Walking
Social media users are talking about lymphatic walking, which combines slow, intentional steps with diaphragmatic (belly) breathing and sometimes arm movements. Lymphatic walking is easy to do and is accessible for most people, says Bobby Pourziaee, DPM, a board-certified foot and ankle surgeon and founder of Rodeo Drive Podiatry in Beverly Hills, California. "It can be particularly helpful for people who spend long periods sitting at a desk, traveling, or otherwise remaining sedentary. Even a relatively short walk can help get the muscles working again and reduce the heavy, stiff feeling that often develops after prolonged sitting," says Dr. Pourziaee.
Dr. Bobby Pourziaee
People with known heart, kidney, vascular, or significant lymphatic conditions should also speak with their healthcare provider about the appropriate type and amount of exercise for them, Pourziaee says. There is no research to support claims that this walking style offers benefits beyond regular walking for maintaining a healthy lymphatic system.

PODIATRY AND SPORTS MEDICINE
Sports Podiatry Clinics Project Growth to $2.29 Billion by 2030
The sports podiatry clinic sector is experiencing rapid growth due to increasing sports participation and rising awareness about foot health. This expanding field is vital for athletes and active individuals, offering specialized care to prevent and treat sports-related foot and ankle injuries.
Projected Market Size and Growth Trajectory of the Sports Podiatry Clinic Market
The sports podiatry clinic market has witnessed significant expansion in recent years. It is expected to rise from $1.54 billion in 2025 to $1.67 billion in 2026, reflecting a compound annual growth rate (CAGR) of 8.1%. The historical growth is largely driven by factors such as increased participation in both amateur and professional sports, limited awareness about biomechanical foot health, a growing number of sports-related lower limb injuries, the proliferation of physiotherapy and rehabilitation centers, and the rising prevalence of obesity and sedentary lifestyles.

RELEVANT RESEARCH
GLP-1 Drugs Linked to Ankle Arthritis in Diabetics
A retrospective database study published in Foot and Ankle Orthopaedics found that GLP-1 receptor agonists, such as semaglutide and liraglutide, are associated with a higher risk of developing ankle osteoarthritis in adults with type 2 diabetes. Notably, this elevated risk did not appear in patients who were obese without diabetes, nor did it result in an increased need for ankle surgery. These findings contribute to a broader, somewhat contradictory body of research regarding GLP-1 medications and joint health, where separate studies have noted benefits like fewer emergency room visits post-surgery alongside slight increases in conditions like gout or osteoporosis.
Because retrospective observational studies show correlation rather than proof of direct causation, medical experts emphasize interpreting the findings with caution. Patients taking GLP-1 drugs in the study had worse overall baseline health, and frequent doctor visits may have simply led to higher rates of arthritis diagnosis. Individuals taking GLP-1 medications for diabetes should not stop their treatment based on a single study; instead, those experiencing new ankle pain, stiffness, or swelling should report their symptoms to a doctor while continuing supportive joint care through proper footwear and exercise.

SOLE PURPOSE BY SAVANNAH SANTIAGO, DPM
Building a Birdhouse, Building a Surgeon
Here are some valuable tips that have aided Savannah Santiago, DPM in navigating her experiences as a student and a resident:
4. Things Might Not Go as Planned (They Likely Never Do)
I went to Home Depot, bought my wood, and confidently made my first cuts—only to discover my dimensions were slightly off. One piece didn’t sit perfectly flush. A few nails went in at questionable angles and had to be removed and redone. Whether it’s a nail gun or a K-wire driver, angles and spatial awareness are some of the hardest skills to master. My dad did mention that since starting residency my “negative pressure” when hammering has been impressively controlled. He is my father, so he is contractually obligated to believe I am excellent. Still, the lesson is being held.
In the OR, complications require clear communication, composure, preparation, and flexibility. If something significant happens, you must communicate effectively with everyone in the room and maintain a level head, whether the issue stems from your own error, someone else’s, or simple unpredictability. Most importantly, you should anticipate that something may not go perfectly and have a contingency plan ready—not only intra-operatively but post-operatively in clinic as well.

RESPONSES/COMMENTS (PODIATRIC PRODUCTS IN THE NEWS)
RE: Treatmedy Introduces At-Home Bunion Device
From: Rod Tomczak, DPM, MD, EdD
There is a current website that I’m sure many of you have seen advocating an apparatus that is a “do it yourself home cure” for painful bunion deformities. The date of the report is March 16, 2026 and the name of the plastic and metal gizmo is the Evelmont Bunion Fix. The inventor of the gadget, we are told, is Dr. Blane Schilling, who has no affiliation with the APMA probably because he is not a podiatrist. His place of practice is unknown. I did find a Blane Schilling PT, MD in Colorado who advertises himself as a “wellness expert” who has discovered the ice water and aquasculpt for weight loss treatment, a triple fusion massager for sciatica, and a “Foot Renew” which is a three layered strap-on appliance that initially offers 6-12 hours of relief from neuropathy after 15 minutes of treatment combining thermal rejuvenation, active massage and dynamic compression. In addition, he is a huckster for the Evelmont bunion fix. All these websites look remarkably similar.
On the neuropathy website, he is a neurologist with 40 years of experience, on the aquasculpt site he is a physical therapist with 40 years of experience. He says he has been a podiatrist for over three decades but does not give us an exact number; maybe because he isn’t a podiatrist. But he does say he is about to piss off...
Editor's note: Dr. Tomczak's extended-length letter appears here.

RESPONSES/COMMENTS (CODING CORNER)
From: Michael G. Warshaw, DPM
While lecturing at the ACPPS Conference last weekend, if I said that the 2027 ICD-10-CM changes do not go into effect until January 1, 2027, I apologize. The new, revised and deleted codes go into effect on October 1, 2026.
The key issue is whether the bilateral excessive pronation is a medically necessary condition requiring correction in both feet, rather than simply providing a second orthotic for symmetry.
For example:
• Right foot: plantar fasciitis (As of October 1, 2026: M67.A01) + excessive pronation (M21.6X1- Other acquired deformities of the right foot).
• Left foot: excessive pronation (M21.6X2- Other acquired deformities of the left foot), but no plantar fasciitis.
• If the examination documents that the left foot has a biomechanical abnormality that requires orthotic correction, a pair can be medically reasonable.
• If the documentation only establishes a problem in the right foot and gives no clinical justification for treating the left foot, a payer may question why two devices are necessary.
For commercial insurance, coverage is plan-specific. Some plans will cover a pair when bilateral biomechanical abnormalities are documented even though the symptomatic diagnosis is unilateral. Please confirm this in advance with the commercial health insurance contractor.
From a documentation standpoint, document the findings separately for each foot:
Right: Plantar fasciitis with excessive pronation and associated biomechanical dysfunction.
Left: Excessive pronation with documented biomechanical dysfunction contributing to abnormal gait/loading.
Assessment: Bilateral biomechanical dysfunction requiring bilateral orthotic control; although plantar fasciitis is currently symptomatic on the right, correction of the underlying bilateral pronation is medically necessary. That is much stronger than simply documenting "right plantar fasciitis—dispense bilateral orthotics."
Michael G. Warshaw, DPM, CPC, Mount Dora, FL
