NY Podiatrist Discusses Ingrown Toenails
An ingrown toenail happens “when the nail plate [the toenail itself] impinges or sticks into the surrounding flesh of the toe,” explains Michael J. Trepal, DPM, a practicing podiatrist and the Dean of the New York College of Podiatric Medicine. It can affect any toe, but the big toe is affected most often because it sits closest to the edge of your shoe.
Dr. Michael Trepal
Three things put you at higher risk: 1) Genetics. “Some people have what we call a pincer or curved nail plate,” Dr. Trepal says. “Instead of growing flat, these nails grow at an angle, causing them to impinge or stick into the tissue surrounding the nail bed.” 2) Tight shoes. Narrow toe boxes squeeze toes together, and runners, hikers, and skiers are especially prone because of their gear. 3) Rounded trimming. Curving the edges of your nails encourages them to grow back into the skin. Trim straight across instead.

PODIATRISTS AND HUMANITARIAN EFFORTS
WA Podiatrist Leads Surgical Mission to India
Lalita Turaga, DPM, a board-certified foot and ankle surgeon, and her former residency director, Martin Yorath, DPM, completed a surgical and teaching mission at the Balaji Institute of Surgery, Research, and Rehabilitation for the Disabled (BIRRD) in Tirupati, India, August 16-21, 2026. The team ran outpatient clinics, screening roughly 100 patients. They performed surgical cases, treating neglected clubfoot and congenital vertical talus deformities, neglected trauma including ankle fractures, and chronic tendon injuries. Both also taught educational sessions for BIRRD’s orthopedic residents.
(L-R) Drs. Martin Yorath, Gudaru Jagadish (Director and Senior Orthopedic Surgeon; Venugopal T (Senior Orthopedic Surgeon; Lalita Turaga (Founder, Souls for Soles), flanked by the orthopedic residents of BIRRD Hospital.
It was Dr. Turaga’s second mission to BIRRD, one of several in India over the past decade. They plan to return in 2027 to develop the collaboration into a sustained teaching program. The trip followed Souls for Soles — A Solfège Soirée, a benefit concert Dr. Turaga organized in Bellevue, Washington, in July, supporting her missions abroad and foot care for Seattle’s unhoused.

FL Podiatrist Discusses Study Showing 72% Women Wear Wrong-Sized Shoes
A new study published in the International Journal of Fashion Design, Technology, and Education suggests that you might want to rethink defaulting to your usual size. When looking at U.S. sizing, 72% of participants did not wear the proper shoe size. Shoe sizing isn’t a perfect science, and it tends to focus more on foot length than width, which is where fit issues tend to arise. “Women with essentially the same foot length could differ by as much as 40 millimeters, or about 1.6 inches, in circumference around the widest part of the foot,” explains Daniel Stewart, DPM.
Dr. Daniel Stewart
“I actually wouldn’t say [women] are necessarily choosing the ‘wrong’ size,” Dr. Stewart adds, regarding the mismatch between shoe sizes and actual foot length in the study. “The bigger issue is that shoe sizing is overwhelmingly based on length, while feet are three-dimensional.” So, as the study found, in order to account for tighter widths, many women end up moving up a size—making the length a little too long for their feet.

RE: Asheville Podiatry Associates Closes its Doors
From: Gretchen A. Lawrence, DPM
After 41 years of keeping the people of Asheville, NC on their feet, Drs. Doug Milch and Debra Wright, closed their practice, Asheville Podiatry Associates. It’s the end of an era, and we wish the doctors much rest in their retirement as now every day can be a Labor Day for them! Dr. Doug Milch graduated from the California School of Podiatric Medicine. He completed a surgical residency and followed up with a fellowship in biomechanics; he loves all things about gait! He taught biomechanics at the Temple University School of Podiatric Medicine. His undergraduate training was in exercise physiology and cardiology.
His love for running led him to complete the Boston marathon twice. He chose podiatry after coming down with a case of Achilles tendinitis while on the football team in high school. He then got his first pair of orthotics and his history in podiatry unfolded. Dr. Milch was published in JAPMA in 1986, writing about rehabilitation of inversion ankle sprains. He was one of the first remaining private practicing podiatrists to gain surgical privileges at Fletcher hospital, now Advent Health Hendersonville in Hendersonville, NC.
We thank Drs. Doug Milch and Debra Wright for a job well done and are blessed for the footprint they made for other podiatrists and the future of podiatry in our great Tar Heel state!

RESPONSES/COMMENTS (SOLE PURPOSE BY SAVANNAH SANTIAGO, DPM)
From: George Vito, DPM
Dr. Santiago hit the bullseye in reference to surgical training and success of the surgeon. While as an attending at North Lake Regional Medical Center in Tucker, Georgia, I had the opportunity to interview residents through the selection process. The applicants knew of my reputation of asking rapid-fire anatomy questions in reference to the origin and insertion nerve innovation and vascular supply. They never really knew the reason why I used such a format until the residents who actually got into the program asked me about it. I tried to explain to them that I did not care what the answer was. I was looking for their emotion and their ability to cope with the stress of not knowing the answer, or knowing the answer but changing the thought process rapidly to another topic, or another question.
The goal was not to answer every question. The goal was to keep a steady pace on focus and to maintain the end game. In surgery, we face many complications; unexpected outcomes. We need to develop the ability to keep moving, even if our previous attempts at a certain technique or fixation method may have failed. I can fully attest that I have had many surgical failures...and hopefully just as many successes. This process can pertain to surgery and to life itself.

From: Allen Jacobs, DPM
With reference to GLP-1 medications and an increased risk of ankle osteoarthritis, the study may be inherently flawed. Patients with type II diabetes have been previously demonstrated to exhibit almost twice the risk of osteoarthritis when compared to non-diabetics. GLP-1 medications may be related to improved outcomes in the management of patients with diabetes and ulceration, including reduced amputation rates and reduced mortality following amputation. Improved wound healing has also been suggested in some patients. The question of whether a podiatrist should employ GLP-1 medications in the management of diabetic patients is an interesting one.
