PMA of NAP Announces Arthur E. Helfand Visionary Medallion

The members of the Podiatric Medicine Academy (PMA) of the National Academy of Practice (NAP) have announced that they will celebrate and encourage outstanding visionary achievement within our podiatric community at the 11 podiatry medical colleges by introducing the “Arthur E. Helfand Visionary Medallion”, an annual award to be presented to a 4th year podiatry medical student at each school for exceptional contributions in podiatry interprofessional research, teaching, community, and leadership engagement in the promotion of the vision of Arthur E. Helfand, DPM.

Arthur E. Helfand Visionary Medallion

Each podiatry medical college will select a qualified 4th year podiatry medical student based on the medallion’s rubric. Dr. Helfand recognized that foot health was linked to overall systemic longevity, mobility, and independence. He served to bridge the gap between podiatry, mainstream medicine, and public health, bringing the fields closer together for the common good and better patient care through an interprofessional approach. Helfand was a founding member of the PMA of the NAP forty years ago. The podiatry academy continues his vision and work towards patient advocacy and intervention.

INTERNATIONAL PODIATRISTS IN THE NEWS
TX Podiatrist Receives International Lifetime Achievement Award
Robert G. Frykberg, DPM, MPH, professor in the Department of Orthopaedics at UT Health San Antonio, received the Diabetic Foot Society of India’s 2026 Lifetime Achievement Award on October 3 at DFSICON in Bangalore, India. The award recognizes nearly five decades of contributions to diabetic foot care and limb preservation, including his work in Charcot foot, diabetic foot ulcers and infections, wound management, off-loading and amputation prevention. It also honors his lifetime of mentorship to diabetic foot specialists worldwide. Frykberg is a past president of the International Association of Diabetic Foot Surgeons and the American College of Foot and Ankle Surgeons.

Following the ceremony, Dr. Frykberg delivered an oration, “The Menace of Diabetic Gangrene,” tracing the history of diabetic limb preservation from Boston, Massachusetts in the 1930s to the present day. His presentation highlighted the continuing challenge that, nearly a century later, gangrene remains a limb-threatening complication of diabetes and frequently leads to amputation.

I could not agree more with Dr. Tomczak. As a matter of fact, I sent an email to the Scholl College about how I felt that their offer for the position was disgusting and offensive to our profession. I recommend that others do this, and let the school know that this is not acceptable.

Sued for a steroid injection? Wow. That says something about the profession, don’t you think?

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1A
From: Amol Saxena, DPM, MPH
Great letter by Dr. Ribotsky! He hit a grand slam with all his points. The naysayers about podiatry needing to change should read it. For sure, you need to have passion for your career. Medicine has changed significantly since Bret and I graduated. The salary being offered to DPMs is probably the same as it was 30 years ago. Is the starting salary as the Chief of Medicine at the same school that low? I doubt it. This shows that even within the same institution, we do not have parity.
My three kids chose three very different paths: D1 Head Coach, orthopedic surgeon, and professional dancer. They have very different incomes but are happy with their choices 10+ years later. I tell those who will listen, "you need to be realistic, not idealistic, nor pessimistic."

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1B
From: Allen M. Jacobs, DPM
Dr. Ribotsky suggests that the principles of medical ethics put forth by Sir William Osler are no longer applicable. Medical debt, Dr. Ribotsky, can explain why a doctor is financially motivated, but does not by itself justify placing profit ahead of patient welfare. High podiatry-school debt and lower early-career income and financial pressures can create incentives for the podiatric physician to employ higher-paying procedures or practice models. The current issue of CAMP payment for wound care is a paragon example. The podiatric physician has a professional obligation to make clinical decisions based upon the patient's medical needs rather than personal financial gain. I guarantee that when Dr. Ribotsky seeks medical care, he wants that healthcare provider to make decisions regarding his care based upon best evidence and need for the patient, not what is in the best financial interest of the healthcare provider. We owe our patients the same.
If our healthcare system has created strong financial incentives that conflict with patient care as Dr. Ribotsky has also suggested, I agree that criticism of the podiatric physician alone does not address the larger issue. Choosing to learn good business or practice management principles, that is, choosing financial stability, is not in and of itself putting profit over patient well-being. The ethical concern arises if financial considerations influence a particular patient's care inappropriately. PM News has always and continues to provide a source for such information.
Debt upon graduation may EXPLAIN a podiatric physician's financial priorities, but Dr. Ribotsky's EXPLANATION IS NOT THE SAME AS JUSTIFICATION. The essential issue addressed by Sir William Osler remains as true today as 100 years ago: the question of whether financial incentives are influencing clinical decisions that should be based primarily on the needs of the patient. The affirmative answer to this is always wrong.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2
From: Steven I. Gubernick, DPM
Regarding Dr. Tomczak’s comments about a lack of options for podiatrists, I disagree. I’ve been working in consulting for 25 years and have advanced through the ranks to a senior position. One classmate is in senior management of a small biotech, while another works in pharma. Life is what you make of it. There are always choices. Podiatry is not the ‘end all be all’ but can be leveraged to finding a successful career.
