PODIATRIC QUICK POLL

At what point do you biopsy a non-healing wound?

AT THE COLLEGES
WUCPM to Expand Virtual Reality Learning Modules and Financial Aid
Western University of Health Sciences’ College of Podiatric Medicine is expanding its virtual reality (VR) operating room thanks to a $500,000 gift from Bruce R. Lawrence, DPM, and Mrs. Janet Lawrence. “The Lawrence family is an extremely generous supporter of advancing education,” said WesternU College of Podiatric Medicine Acting Dean Jacqueline Truong, DPM, MPH.
Dr. Bruce Lawrence
Third-year College of Podiatric Medicine students will begin using the first two VR operating room modules this semester. The first module provides content on common surgical instruments used in podiatric surgery. The second module focuses on lower-extremity anatomy and common incisions in podiatric surgery. Beyond the VR learning modules, Dr. and Mrs. Lawrence have also provided a generous $50,000 endowed scholarship, which will be distributed for the first time during the 2026-27 academic year.

PODIATRISTS AND PODCASTS
Pod of Inquiry Podcast Celebrates 4-Year Anniversary
The Pod of Inquiry, hosted by Stephen Barrett, DPM, celebrated its 4-Year Anniversary with Dr. Aseem Malhotra, an internationally recognized cardiologist, author, and advocate for evidence-based medicine. This wide-ranging conversation went well beyond cholesterol and statins into metabolic health, nutrition, the integrity of the medical evidence base, and the forces that shape what physicians are taught and what patients are told. It is a candid discussion about what the data actually show, and what it will take to restore trust between medicine and the public it serves.
Pod of Inquiry Podcast Celebrates 4-Year Anniversary
Dr. Malhotra joins an already world-class guest list that includes luminaries like Drs. Kevin Tracey, Dale Bredesen, and Robert Lufkin. Now with over 200 episodes, this series covers virtually every specialty in medicine, health optimization, and other cutting-edge medical innovations.

RESPONSES/COMMENTS (PODIATRIC MEDICAL NEWS QUICK POLLS)
From: John J. Doolan, DPM
I think there is more to the question of when to biopsy a non-healing wound. The most important thing is to know if there is a logical or obvious etiology. Of course, even a diabetic can get skin cancer on the bottom of their foot. If there is no known etiology, the answer should be immediately.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1A
RE: Podiatrists Needed; Not Aiding Arkansans
From: Brooke A. Bisbee, DPM via Arkansas Democrat Gazette
As immediate past president of the American Podiatric Medical Association, I have specialized in foot and ankle health for more than 25 years. Doctors of podiatric medicine (DPMs) treat all foot conditions ranging from toenails, bunions, fractures, and plantar fasciitis to serious diabetic foot complications that can lead to limb amputation. Their expertise helps prevent costly complications and improves the efficiency of our health-care system.
I practice in northwest Arkansas, where rapid economic and population growth has outpaced the availability of podiatrists. Statewide, Arkansas has just 67 podiatrists serving more than 3.1 million residents--approximately one podiatrist for every 46,300 people. Rural patients may travel long distances for care, making it harder to receive preventive treatment and regular checkups.
This shortage is especially concerning given Arkansas' diabetes burden. Nearly 14 percent of Arkansans have diagnosed...

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1B
From: Bret M. Ribotsky, DPM
Dr. Jacobs states that I suggested Osler's principles "are no longer applicable." I did not say that, and nothing in my letter can fairly be read that way. I hold those principles as firmly as you do. He then spends most of his response insisting that explanation is not justification. I agree. I never offered a justification. I described a system in which the incentives are structured against the very ethics we both endorse, and I asked why we keep pointing at the individual physician while ignoring the structure that shapes his or her choices.
His second paragraph proves the point. He concedes that criticizing the podiatric physician alone "does not address the larger issue," and that choosing financial stability is not inherently putting profit over patients. That is my position. We are in agreement, so I'm puzzled that it took three paragraphs and a public rebuke to get there. I would also remind him that his "guarantee" about what I want from my own physician is an assumption, and a presumptuous one. I have spent a career insisting that patient need, not reimbursement, drives clinical decisions. Nothing I wrote suggests otherwise.
The profession is better served when we read what a colleague actually wrote, not what we expect or fear he wrote, before holding him up to public criticism. Debating the real argument would have been more useful to our readers. I would welcome that debate, starting with how a reimbursement structure like the current wound care payment issue pushes physicians toward the very conflicts Osler warned against.

RESPONSES/COMMENTS (IN THE COURTS)
From: David S. Wolf, DPM
I respectfully disagree with Dr. Grossman’s conclusion that this case “says something about the profession.” To me, it says far more about the increasingly litigious environment in which all healthcare professionals practice. A malpractice claim arising from a corticosteroid injection is not unique to podiatry; comparable allegations can be brought against any medical specialty.
This case illustrates the safeguards built into the legal process. Also in this case, a medical malpractice tribunal found insufficient evidence to raise a legitimate question of liability, and the plaintiff then failed to post the bond required to proceed with the claim.
Rather than reflecting poorly on our profession, this case is a reminder that podiatrists, like all health professionals, remain vulnerable to malpractice allegations regardless of the ultimate merits of the claim. Expert witnesses (not hired guns for the plaintiffs) and attorneys certainly have legitimate roles in the judicial process, but those roles should always be exercised responsibly and based upon sound medical evidence.
David S. Wolf, DPM, retired
