PODIATRIC QUICK POLL

At what point do you biopsy a non-healing wound?

PODIATRISTS AND SPORTS MEDICINE
CA Podiatrist's 4-Step Routine to Reduce Injury Risk After Running
Follow these simple steps immediately following a longer run, says Bobby Pourziaee, DPM. 1) cImmediately after the run, take your shoes off, let your feet air out, and take off your socks. You’re first going to inspect your feet,” he says. “Look for any blisters or any issues with the toenails.” 2) Try a recovery shoe. “Most of these post-recovery shoes are supportive, and they’re wide, so they help with swollen feet after a run,” he adds.
Dr. Bobby Pourziaee
3) Periodic Icing. “Initially, icing it down could be a good idea. Doing it for about 10 minutes on and 20 minutes off, and doing that for a period of time initially is helpful,” he explains. 4) Put on compression socks. Transition into some type of compression stockings or socks to help reduce swelling. Make sure they extend all the way up to just below the knee rather than stopping midway down the calf, as this helps support venous circulation and encourages blood flow back to the heart. Compression can also provide support to the muscles and may help with post-exercise soreness.

QUERIES (CODING & BILLING ETHICS)
Query: Duty to Provide Services vs. Remuneration
I had a patient come in today with paronychias on three toes on each foot. The patient saw a local podiatrist who said that the patient's insurance only allowed one side to be fixed per appointment. The patient has Blue Shield PPO. The doctor collected $200 for the procedure as the deductible had not been met and told the patient to reschedule for the next P&A to be done. Now we all know that insurance companies pay poorly and do not reimburse for a procedure on the second side at the same visit, but in this case, the patient had real pain and real incurvated nails that needed attention.
Is it ethical to make the patient come back 11 more times, or should I take care of the problem at one sitting? What do the experts say on this?

RESPONSES/COMMENTS (AT THE COLLEGES)
From: Michael Lawrence, DPM
The generosity of my brother, Bruce Lawrence, DPM, and his wonderful wife Janet cannot be overstated. This learning program will benefit students in their clinical training in prolific ways and then follow them through their careers in our profession. For those who don't know, and that would be most everyone, this act represents only a fraction of what they have selflessly given to the profession!
And the beneficence doesn't end with podiatry. They supported a women's and children's shelter in their community with a substantial gift to remodel and update ALL their residential units. How about funding for a cancer research program at the University of California, San Diego? And get this, after Bruce was treated for an eye condition at UCSD, he learned of an outreach program for children, utilizing a mobile clinic, serving those in need. So, being a senior and sensitive to the needs of older adults, Bruce and Janet paid for an EYEMOBILE to serve adults in San Diego County who otherwise would not have access to care! HUNDREDS of seniors have been treated to improve their vision and DOZENS have received vision-saving care! Think about that. Every time I do, my eyes get wet!
I am so lucky, proud as well to call him brother and her sister-in-law.
Michael Lawrence, DPM, Chattanooga, TN
