PODIATRY AND DIABETES

In Remission, Timing Matters

The Appointment is Part of the Treatment: Podiatry Frequency in Diabetic Foot Remission

In Remission, Timing Matters
The review highlights the DIATIME randomized clinical trial, which enrolled 148 high-risk people in remission after a healed plantar foot ulcer and followed them for one year. Participants were assigned to podiatric screening and foot care every 4, 8, or 12 weeks.
The comparison was unusually clean. All three groups received therapeutic footwear with rigid rocker soles and custom-made insoles. All received podiatric care, including management of hyperkeratosis and nail care. The principal difference was cadence: callus care and screening occurred monthly, every two months, or every three months.
The reported one-year plantar-ulcer recurrence rates were:
Every 4 weeks: 18.4%
Every 8 weeks: 28.6%
Every 12 weeks: 46.0%
The between-group difference was statistically significant. In this population, the shortest interval had the lowest recurrence. That is more than a scheduling result. It suggests that, during remission, time itself can become a risk factor.

PODIATRISTS AND FOOTWEAR
Worn-Out Footwear Can Trigger Plantar Fasciitis: TX Podiatrist
Plantar fasciitis can be caused by…a lot of things. Among them are “prolonged standing, changes in activity patterns, tight calf muscles, a shortened Achilles complex, high arches, and flat feet,” says Anne Sharkey, DPM, a double-board-certified podiatrist based in Austin, TX.
Dr. Anne Sharkey
Worn-out running, training, or walking shoes can also be the culprit. Though your shoes may have started out with ample cushion and support, if they’ve worn down, they could be causing your current foot pain. But how do you know if your shoes are worn out? “Look for excessive wear at the outsole with loss of lug/traction pattern,” Dr. Sharkey says. “Also look for loss of responsive cushioning—sometimes the midsole will show excess wrinkling as it wears and loses support.” As a general rule, for moderate walkers, it’s best to replace your shoes every six months (every year at the longest!).

RESPONSES/COMMENTS (PODIATRISTS AND PODCASTS)
From: Savannah Santiago, DPM
I appreciate Dr. Tomczak taking the time to respond, and I enjoyed the history lesson on RICE. That arc is actually a good case for my argument: our understanding evolves, and our profession is better when we evolve with it. I also share Dr. Tomczak's love of running. As a collegiate cross-country and track athlete, I saw the effects of RED-S firsthand long before I knew it had a name. Those experiences gave me my earliest exposure to podiatry, and I'm grateful for that. They're a large part of why I believe our profession has a role to play here. The athletes who end up in our offices with stress fractures are often the same ones nobody has thought to ask about the bigger picture.
That brings me to one clarification. RED-S (Relative Energy Deficiency in Sport) isn't a trendy new name for over-training. The IOC consensus statement, introduced in 2014 and updated in 2023, defines it as the result of low energy availability. It also most certainly doesn’t just impact metatarsals. When an athlete's intake doesn't meet the demands of training, the effects reach bone health, endocrine and menstrual function, metabolism, immunity, cardiovascular health, and psychological well-being. Training volume plays a part, but the core problem is fuel, not mileage.
Dr. Tomczak is right that the foot and ankle is not a system. It is, however, often where a systemic problem...

RESPONSES/COMMENTS (PODIATRISTS AND THE LAW)
From: Jeffrey Kass, DPM
Can someone explain the math behind the podiatrist who committed 11 million dollars in healthcare fraud and had to pay back over 1.6 million dollars in restitution and over 1.3 million in forfeiture? Also, if we can be enlightened on what this 11 million dollar fraud was, I’d be curious to hear.
