IL Podiatrists and SCPM Students Volunteer at Chicago Marathon
The 2026 Chicago Marathon took place on Sunday, October 11th. Podiatry was again well represented as an integral part of the marathon medical team. As in previous years, AAPSM Fellows Jeffrey Baker, DPM of the Weil Foot & Ankle Institute and Michael Chin, DPM of the Running Institute were podiatry co-coordinators for the event.

Other podiatric Volunteers were Ersta Ferryanto, DPM, Kathryn LaViolette, DPM, and Alison Young, DPM. Volunteering as well were podiatric medical students from the AAPSM Scholl Chapter. The podiatry volunteers were stationed at two medical tents at the finish line area and provided medical care to 185 runners throughout the day.

Part B MACs Propose Non-Coverage for Fluorescence Wound Imaging
Proposed local coverage determinations from all 7 Part B Medicare Administrative Contractors would designate real-time fluorescence wound imaging for bacterial presence as a non-covered service. Stakeholders can submit comments through November 14, 2026, before the contractors move toward final coverage decisions.
Key Takeaways
  • The policies are proposed, not final. All 7 Part B MACs released identical non-coverage proposals on October 1, 2026.
  • The MACs contend that there is insufficient evidence that real-time fluorescence wound imaging independently diagnoses wound infection, accurately determines bacterial burden, or improves patient health outcomes compared with standard assessment and management.
  • Stakeholders can participate in the comment process through November 14, 2026. Submissions challenging the proposals should include peer-reviewed evidence addressing the MACs’ stated concerns.

RESPONSES/COMMENTS (CLINICAL) - PART 1A
From: Jack Ressler, DPM, David Secord, DPM
Dr. Byron’s post brings up excellent points. Taking biopsies can often be overlooked but should be the standard of care when treating patients for skin lesions, especially a possible verruca. Several years ago, I attended a lecture given by Bako diagnostics. The lecture consisted of a slide show with a lecturer who pointed out that every patient in his presentation had died. Several patients were not elderly and many were originally being diagnosed and treated for a verruca without a biopsy being taken. Enough said!
Jack Ressler, DPM, Boca Raton, FL
For the treatment of mosaic warts, I recommend using the Panacos graft, a one-time procedure. I only use bleomycin for individual verruca vulgaris lesions, the beauty of which is that treating one lesion stimulates the production of killer T cells to the virus and in 5 weeks or so, every single wart on either the hands or feet will disappear. This is a wonderful procedure in those with normal immune systems.

RESPONSES/COMMENTS (CLINICAL) - PART 1B
From: Michael Michetti, DPM
I read Dr. Byron’s post regarding the treatment of verrucae and felt his protocol is unnecessarily elaborate and contributes to the increasing cost of medical care. A podiatrist is trained to recognize a verruca clinically, and I see no reason why it cannot be removed at the initial visit, with the specimen then sent to pathology if there is any diagnostic uncertainty or if the podiatrist feels it is warranted.
Dr. Byron’s approach also seems to eliminate the use of topical chemical agents, such as Cantharone or salicylic acid, which have long been used in the treatment of warts and are well within the standard of care. During my years in practice, routine follow-up care after curettage often consisted of a Band-Aid and topical antibiotic cream.
I believe his protocol is over-the-top for what is often a relatively simple podiatric problem. We should be mindful of providing appropriate care without adding unnecessary steps, expense, and inconvenience for the patient.
Michael Michetti, DPM, Gaithersburg, MD

RESPONSES/COMMENTS (CLINICAL) - PART 1C
From: Allen M. Jacobs, DPM
Dr. Byron recalls his pain as a 10-year-old child being treated for a wart. He then describes his protocol for surgical excision of a wart and follow-up "wound care", the need for a surgical shoe, and post-operative care. There is an entire industry built upon treatment of warts in children, ranging from cryotherapy to various lasers, to surgical excision.
The reality is what the actual literature demonstrates, and what it demonstrates is that aggressive treatment of warts in children and adolescents is seldom indicated. Parents should be informed that watchful waiting is appropriate, particularly for non-painful verrucae. Spontaneous resolution of warts in children occurs in 50% within one year, 65% in 2 years, and 95% within 4 years with NO TREATMENT. Furthermore, topical salicylic acid has the greatest literature support for warts in children as effective, inexpensive, painless, and safe with few side-effects.
That is reality, corporate-induced [protestations to the contrary]. The majority of industry-supported, alternative therapies demonstrate effectiveness compared to...

RESPONSES/COMMENTS (PODIATRISTS AND PODCASTS)
From: Ben Pearl, DPM
I read with interest the dialogue regarding a podiatrist’s role and conditions like RED-S. As a colleague, I thought the topic was relevant at the time I had her on my podcast, and was even more impressed once she presented her talk. Being aware of factors like calorie count, Vitamin D, and albumin levels is critical to optimize our patient care.
The discussion also raises broader themes about the scope of podiatry. I don’t see how you can properly treat neuropathy, for example, without understanding methylation and its role with absorption of Vitamin B12, folate, and other co-factors like antioxidants. The same holds true with the interplay of systemic considerations for wound healing. Whether we decide to initiate treatment or pass off the baton to an appropriate internal medicine doctor or specialist depends on access to the referral, urgency of that patient's needs in real time, and diligence. The individual practitioner has demonstrated proficiency in the discipline that they are screening or treating.
The knowledge to be diligent requires more than just superficial understanding of medicine and physiology to stay relevant as a profession.
