PODIATRIC QUICK POLL

At what point do you biopsy a non-healing wound?

AT THE COLLEGES
SMU-CPM Students Deliver Free Care at "Suitcase Clinics"
Every few Tuesdays throughout the school year, Samuel Merritt University College of Podiatric Medicine (SMU-CPM) students volunteer at free community clinics hosted by UC Berkeley pre-med students, providing basic podiatric care, footwear, and wound-care support to under-resourced and unhoused patients in Berkeley, CA. These “Suitcase Clinics” give Doctor of Podiatric Medicine students supervised, hands-on experience before rotations, teaching them to navigate a patient visit with standard practices like listening, asking questions, examining and ultimately treating through tasks like diabetic foot exams, toenail trims, callus debridement, and wound care.
SMU-CPM Students Deliver Free Care at "Suitcase Clinics"
“It gives us a chance to experience what it means to treat this kind of community,” reflects Connor Jow, third-year DPM student and president of SMU’s American Public Health Association (APHA) club, which organizes SMU CPM’s involvement in these clinics. “These patients haven't seen a doctor in a while…maybe ever. It’s our chance to give that human connection and help them feel comfort in the treatment we give them.” Colleen Kenney, DPM, often supervises DPM students at Suitcase Clinics as their physician attending. A California College of Podiatric Medicine (SMU CPM’s previous name) ’85 alumna, Dr. Kenney has been involved in these clinics for the past ten years. “Teaching has been an important part of my professional life,” she says.

PODIATRISTS AND SURGERY - PART 1
Bunion Surgery Can Improve the Quality of Life: CA Podiatrist
“Bunions usually get worse as time goes by,” said Robert Lee, DPM, chief of podiatric foot and ankle surgery at the University of California, Los Angeles, Santa Monica Medical Center, but they don’t always get bad enough to require an operation. Whether you develop bunions largely depends on your genes, Dr. Lee said. While the bump isn’t there when you’re born, “you’re actually inheriting a foot type that’s prone to forming the bunion,” Dr. Lee said.
Dr. Robert Lee
If your pain is getting worse or is affecting your daily routine, the experts said you should consider seeing a podiatric surgeon or orthopedic surgeon who specializes in the foot and ankle and has experience performing bunion surgeries. Dr. Lee said that compared with a few decades ago, surgeries now involve smaller incisions and cause fewer complications. While surgery still often requires months of recovery, he said, it “can really improve quality of life” for those in significant pain.

PODIATRISTS AND FOOT SURGERY - PART 2
What Patients Having Foot Surgery Need to Be Told: CA Podiatrist
"Every week, a patient sits in my chair fixated on one question: 'How long until the stitches come out?' That is the wrong question. The stitches are the easy part. What I wish more patients understood going in is that the visible healing on the outside of the foot and the actual healing happening underneath, in the bone, the joint capsule, and the soft tissue, run on two completely different timelines, and the gap between them is where most of the frustration lives," says Dana Figura, DPM
Dr. Dana Figura
"Swelling is the biggest one [issue]. After a bunion correction, hammertoe repair, or similar procedure, I tell patients to expect noticeable swelling for three to six months, and a lingering, low level puffiness by the end of the day for up to a year in some cases. The second thing rarely mentioned is stiffness. A joint that was surgically re-aligned needs to be moved, gently and deliberately, or it will tighten in whatever position it healed in. The third is emotional. Being off your feet, dependent on a boot or knee scooter, and unable to run normal errands for six to eight weeks wears on people in a way nobody warns them about beforehand," says Dr. Figura.

PODIATRISTS AND SPORTS MEDICINE
CA Podiatrist Discusses Haglund’s Syndrome and Achilles Tendinopathy
Haglund’s syndrome and insertional Achilles tendinopathy typically develop and worsen slowly. The causes aren’t completely clear. Genetics seem to play a role in the bony growth itself, says Amol Saxena, DPM, MPH, a surgeon in private practice in Palo Alto who has operated on hundreds of Achilles tendons.
Dr. Amol Saxena
Research also suggests that extracorporeal shockwave therapy (ESWT) can provide relief, although it isn’t always covered by insurance, Dr. Saxena says.

RESPONSES/COMMENTS (CLINICAL) - PART 1A
From: Lawrence M. Rubin, DPM
Dr. Michetti raises a critical point that deserves thoughtful attention across our profession. I strongly agree that routine biopsy for clinically classic verrucae is very often medically unnecessary. As podiatrists, we are highly trained clinical diagnosticians, and in the vast majority of cases, a thorough clinical evaluation of the lesion or lesions is more than sufficient to establish an accurate diagnosis and initiate appropriate treatment—including long-proven topical agents like salicylic acid or Cantharone at the initial visit.
Beyond the added physical inconvenience and expense for the patient, routine tissue submission for unambiguous cases exposes practices to heightened scrutiny from insurance auditors and third-party payers. Utilization review boards increasingly view routine biopsies for benign, classic clinical presentations as over-utilization rather than high-value care. Biopsy remains an invaluable tool when lesions present with atypical features, recalcitrance, or genuine diagnostic uncertainty, but making it a routine step for every commonplace verruca unnecessarily elevates the burden of care.
We must remain mindful stewards of both patient outcome and healthcare efficiency, ensuring our management plans reflect sound clinical judgment without adding redundant steps or potential financial risk.
Lawrence M. Rubin, DPM, Las Vegas, NV

RESPONSES/COMMENTS (CLINICAL) - PART 1B
From: Chuck Langman, DPM, Jon Purdy, DPM
I no longer treat warts, but for years used intralesional injection of candida albicans antigen (Candin). I found it to be effective, and it usually only required one or two treatments. It led to the single most satisfying case I ever had. I treated a young boy, about 8 years old, with it. He also had warts on the skin around both of his eyes. The doctors at CHOP wouldn’t treat the ones near the eyes. My Candin injection eradicated all of his warts. Needless to say, his family was very appreciative.
Chuck Langman, DPM, King of Prussia, PA
When something works 99% of the time, why would one change? For the last twenty-five years, I have used Cantharone regular or plus every two weeks. The patient applies Sal acid one to two times daily. Usually after the third treatment, we are finished. Some aggressive types take longer. There is the occasional blister which speeds up the process, but overall, it's a fairly painless procedure.
With an aggressive mosaic type presentation, I have the patient use Aldara daily. I have on occasion used Tagamet tid with some great results, usually in the younger population. The last resort is laser excision, but I can’t remember the last time I sent someone for that, and not all insurances cover it. I reserve biopsy for recalcitrant cases or any lesions that appear unusual on initial presentation.
Jon Purdy, DPM, New Iberia, LA

From: Allen Jacobs, DPM
The recent discussion regarding the biopsy of all verrucous-appearing lesions for potential malignancy is an important issue when a thought-leader in our profession suggests this should be the mythical "standard of care". I have always been haunted by the phrase "more mistakes are made by not taking a biopsy than when taking a biopsy." It reverberates in my small brain anytime I diagnose any soft tissue or bone lesion without doing a biopsy.
With specific reference to a plantar verruca, I consulted AI with the question "what are the chances of a plantar wart being a malignant lesion?" The following was the answer which I obtained.
True malignant transformation of a genuine plantar wart is exceedingly rare — benign cutaneous warts caused by common HPV types "almost never undergo malignant change". The more clinically important issue is not that a wart becomes cancer, but that a malignancy (most often verrucous carcinoma, a low-grade squamous cell carcinoma) can masquerade as, and be misdiagnosed as, an ordinary plantar wart.
Why the risk from a true wart is negligible
• Plantar warts are benign intra-epidermal proliferations caused predominantly by low- oncogenic cutaneous HPV types...
